Job Title: RN – DDD Habilitation Nurse
Location: Flagstaff, AZ
Type: Contract
Schedule: Monday – Friday | 8:00 AM – 5:00 PM | 45 Hours Per Week
Shift: Day Shift
Work Setting: Field-Based / Travel Required
Pay: $30/hr - $35/hr (Based on Experience)

Position Overview:

We are seeking an experienced Registered Nurse (RN) to provide nursing support for DDD members with skilled nursing needs. This is a floating/relief role supporting the District North Area based on operational needs.

Responsibilities

  • Conduct comprehensive nursing assessments for DDD members.

  • Complete in-person assessments every 90 days and as needed.

  • Conduct field-based visits throughout the assigned district and statewide when required.

  • Assess members' health conditions and determine appropriate skilled nursing needs.

  • Document medical necessity for skilled services.

  • Monitor members' health needs and coordinate appropriate referrals.

  • Complete nursing referrals and communicate with vendors regarding services.

  • Collaborate with healthcare providers, vendors, external agencies, members, and support systems.

  • Participate in multidisciplinary and interprofessional planning meetings.

  • Advocate for members' rights and access to appropriate services.

  • Complete clinical documentation within established DDD timeframes.

  • Provide temporary nursing coverage when district nurses are unavailable.

  • Complete required meetings, training, and nursing-related assignments.

Required Qualifications

  • Current and active Arizona RN license in good standing.

  • Minimum 2 years of recent RN experience.

  • Experience in Home Health, Hospice, Long-Term Care, or Nursing Case Management.

  • FPCC required.

  • Strong nursing assessment and care coordination skills.

  • Excellent written and verbal communication skills.

  • Strong documentation and organizational skills.

  • Ability to work independently in a field-based environment.

  • Ability and willingness to travel for in-person assessments.

  • Proficiency with Google Workspace/Office, Microsoft Office, and Adobe Acrobat.

  • Ability to use electronic documentation systems and other required software.
