Job Description

MEDICAL SCRIBE (part-time): UBMD Primary Care is seeking a part time Medical Scribe for busy outpatient clinics in Tonawanda, Williamsville and downtown Buffalo at the Conventus Medical Office Building. Qualified candidates should have advanced typing skills, experience with EMRs preferred and knowledge of medical terminology. Medical students and medical assistants are encouraged to apply! Prior experience in a medical office is helpful. Must be willing to travel to all locations as needed. Hours will be Monday through Friday with the opportunity to work 1-2 evenings per week. Must be willing to travel between sites and have appropriate transportation. Parking is paid for downtown location.

Pay range is $16.50-$17.50/hour. Part-time 22.5 hours/week. Any offer of employment is contingent upon successful background check and drug screen. AA/EOE

Job Summary:

The Medical Scribe will be responsible for ensuring proper documentation of clinical services when the billing provider has elected to utilize scribe services. A scribe must be present during a physician’s clinical encounter and documents the physician’s report during the course of the service provided. A scribe must not see the patient in any clinical capacity and must not interject their own observations and/or impressions.

Essential Functions:

  • Responsible for documenting reason for patient visit (HPI – history of present illness).
  • Responsible for updating patient charts including demographic, family, social and medical histories and ensuring annual updates are entered.
  • Responsible for transcribing details of the physical exam and patient orders including any lab tests, imaging tests or medications ordered by the physician. May also be present to record a physician’s consultations with family members or other physicians about a specific patient’s case.
  • Documents procedures performed by the physician or any other healthcare professional, including nurses, physician assistants and/or nurse practitioners.
  • Checks the progress of and reviews lab, x-ray and other patient evaluation data for comparison and transcribes the results into patient charts so that a patient’s information is complete and the physician can make sound treatment decisions.
  • Records physician-dictated diagnoses, prescriptions and instructions for patient discharge and follow up.
  • Records a provider’s consultations with other healthcare professionals, patients and family members.
  • Ensures that signature requirement procedures are followed including name, title and dating of all entries into medical record. The role and signature must be clearly identifiable and distinguishable from that of the healthcare provider. Healthcare provider must authenticate the entry by signing, dating and recording the time through clinical information system. (Stamp is not permitted for use in authentication of scribed entries.) Authentication must take place before healthcare provider and scribe leave the patient care area and cannot be delegated to another healthcare provider.
  • If scribe enters orders into medical record, those orders cannot be acted on until authenticated by the healthcare provider who placed the orders. Authentication includes the healthcare provider’s signature (electronic or manual) and the date/time.
  • Reviews and addresses daily tasks as assigned.
  • Complies with all OSHA regulations.
  • Adheres to HIPAA and confidentiality policies and procedures.

Other Functions:

  • Reviews company email on at least a daily basis in order to receive and address notifications and/or other pertinent information in a timely manner.
  • Any other duties as requested or assigned by the Quality Coordinator and/or other management representative. Duties, responsibilities and activities may change or new ones may be assigned at any time with or without notice.
  • Required to travel to UBMDFM location(s) dependent on company need.
  • Work hours may fluctuate depending on company/clinic needs.

Qualifications:

Education: Must be attending school or be an established medical student, nurse practitioner, physician assistant, registered nurse, licensed practical nurse or medical assistant.

Experience: Minimum of 1 to 2 years’ experience in a healthcare position (medical assistant, nurse, etc.) required. Experience as a medical scribe preferred. Experience with electronic medical records preferred.

Knowledge, Skills & Abilities: Medical terminology required. Excellent communication, interpersonal and organizational skills required. Strong attention-to-detail skills required.

Working/Environment Conditions:

  • Position is in a well-lighted, fast-paced, clean office environment.
  • Office noise level will be mild to moderate most times.
  • Moderate/average indoor temperatures.
  • Travel to various sites via use of personal vehicle.

  • Prolonged periods of sitting or standing at a workstation in order to perform scribe duties on a computer.
  • While performing the duties of this job, the employee will be regularly required to sit, talk, hear and use hands and fingers to operate a computer and/or telephone keyboard.
  • Specific vision abilities required by this job include close vision requirements due to computer work.
  • Light to moderate lifting (up to 15 pounds) may be required.
  • Regular, predictable attendance is required.
  • Must possess the physical and mental abilities to perform the tasks normally associated with the essential job functions such as stationary, mobility, operating machinery, convey and exchange information.

Equipment:

  • Manual dexterity to operate standard office machines/equipment such as computers, printers, multi-line phone, photocopier, fax system, scanner and/or calculator.
