Job Description

Physician Assistant

I. QUALIFICATIONS: A. Graduate of an accredited training program for Physician Assistants. B. Certified by the state of Oklahoma to practice as a Physician Assistant. C. Eligible for medical malpractice insurance. D. Physical and mental capability to perform the responsibilities of the position. E. Maintains current continuing CME requirements and other credentialing and privileging criteria required for this position. F. Possesses DEA, and Oklahoma State Bureau of Narcotics and Dangerous Drug Control Certificate.


II. RESPONSIBLE TO: CMO


III. RESPONSIBILITIES: a. Clinical: i. Provides direct patient care, which includes obtaining a thorough and appropriate history, accurately performing the appropriate clinical exam, arranging testing, therapy, and follow-up. ii. Documents each patient encounter in the patient record, clearly and concisely, on the day the patient encounter takes place. iii. Consults supervising or alternate physician when appropriate. iv. Initiates patient referrals as necessary, and communicates with the referral clinician or agency. v. Maintains and improves clinical skills by attending health center provider meetings, journal clubs, internet, and other meetings. vi. Other responsibilities assigned by the CMO. b. Administrative: i. Participates in provider meetings, quality assurance committees, and staff meetings. ii. Serves as a teacher of other health professionals and as a clinical community leader.


Nurse Practitioner


I. QUALIFICATIONS: A. Registered nurse, currently licensed to practice in State of Oklahoma. B. Certification from a formal Nurse Practitioner Program. C. Experience in ambulatory care setting. D. Interest and/or experience in collaboration with interdisciplinary health teams. E. Maintains current continuing medical education (CME) and criteria for credentialing. F. Possesses DEA, and Oklahoma State Bureau of Narcotics and Dangerous Drug Control Certificate.


II. RESPONSIBLE TO: CMO


III. RESPONSIBILITIES: Characteristic Activities: • Participates in the provision of first contact care and coordination of continuity of care to clients of all ages in collaboration with GSPHC physicians. • Assumes responsibility for the care of a group of clients in collaboration with GSPHC physicians. • Participates in the coordination of community agencies and/or resources in providing continuity of care. • Participates in the continuing education of the nursing staff. • Conducts and/or participates in-group educational experience for clients and/or families. • Participates in individual client or family education or counseling activities in collaboration with other members of the medical interdisciplinary team. • Participates appropriately in staff activities. Specific Functions: • Elicits a chief complaint, history of present illness, patient profile, past medical history, family history and review of systems. This includes the social, economic and emotional data. • Performs a physical examination using the skills of auscultation, percussion, palpation and inspection. • Establishes a problem list and plan of therapy which includes teaching and counseling, ordering and performing laboratory and diagnostic tests for baseline data, diet, pharmacotherapy, activity using problem-oriented medical record system. • Determines final disposition of clients in collaboration with center physicians. Makes appropriate referrals to other resources when needed. • Provides follow up care for monitoring health and disease states, evaluating progress of each problem, and including patient education. • Provides emergency care within limitations. • Participates in clinical audit. • Serves with GSPHC physicians in hospital follow-up of clients in collaboration with the other members of the medical interdisciplinary team. • Performs other duties assigned by the CMO.
