Medical Social Worker (Per Diem)

We are looking for dedicated Social Workers that would provide amazing home health care to all their patients in the Eastbay area of California.

We offer Per Diem Rate of $125 per visit + the Full IRS of $0.67 cents a mile (mileage reimbursement)

ABOUT THE POSITION:

The Medical Social Worker is responsible for the provision of direct social work service in Home Health utilizing the professional healthcare process of assessment, problem identification / planning, action / intervention and clinical goal/outcome evaluation. 

• Provides rehabilitative and supportive casework geared to restoring patients to their optimum level of social, emotional and health adjustment. This includes assisting patients and their families to understand, accept and follow medical recommendations.
• Help patients utilize the resources of their families and the community. This may be accomplished by either referring the patients to resources or acting as an intermediary on behalf of the patients in their dealings with other health and welfare agencies.
• Assists patients and their families with personal and environmental difficulties, which predispose them toward illness or interfere with obtaining maximum benefits from medical care. These range from counseling members of them patients ’s family to assisting clients with admission to a nursing home.
• Consult with the physician and other members of the health team for the purpose of assisting them to understand significant social, emotional and environmental factors related to the patients’ health problem.
• Prepares clinical/progress notes on the day of the visit and incorporates same in the clinical record weekly; provides summaries and re-evaluations if indicated.
• Attends care conferences, initiates interdisciplinary referrals and participates in care coordination as needed.

A Master's Degree in Social Work, CPR/BLS Certification and current Negative TB Test are required for the position.
