The Company: Pathways Vermont is a dynamic nonprofit agency committed to permanently ending homelessness and offering innovative mental health services through ongoing supportive social work.
The Role: The “Medical Provider” is an integral of the Pathways Vermont Leadership team. They will work as part of a statewide modified Assertive Community Treatment team, under the supervision of our Medical Director. Medical providers will provide diagnostic evaluation and direct psychiatry services including medication consultation and management to service recipients in several Pathways programs including Housing First, Soteria House, and the Forensic Assertive Community Treatment Team. The medical provider will also review service plans and service course delivery, and consult with the team and outside providers as needed to ensure high quality and values aligned support for participants in the program.

Schedule: Part-time (Negotiable, estimated 5-10 hours per week)

Support and promote person-centered, trauma-informed care; harm reduction approach
Engage in clinical assessments, medication management, and psychotherapy with clients
Timely and accurate documentation in accordance with Medicaid standards in electronic medical record
Work closely with the ACT team in developing, implementing, amending and approving service plans
Participate in weekly ACT team meetings and case conferences; provide consultations, and make home visits with team members as needed (if local)
Collaborate with nursing & administrative staff to coordinate prescriptions/orders, follow up visits, and to oversee the health of clients
Establish a work schedule, in collaboration with the team, that ensures ongoing psychiatric/medical support for all participating clients on a biweekly, monthly or quarterly schedule
Engage identified clients in psychiatric services, virtually, onsite and/or in the community (if local)
Participate in facilitating appropriate hospitalizations, crisis interventions, and referrals
Communicate with hospital staff to ensure optimal treatment and discharge planning
Provide coverage for other agency medical staff during vacation/sick time
Attendance at the annual All Staff and Programmatic Retreats is an expectation of employment with Pathways Vermont, unless excused by the employee’s manager/HR. Retreats are held on workdays, during normal working hours, and are fully paid.

Qualifications:
Licensed Psychiatric Nurse Practitioner, Licensed Psychologist, Licensed Clinical Mental Health Counselor, Licensed Independent Clinical Social Worker
Licensed or eligible to practice in Vermont
Co-Occurring disorders competent
Ability to work within a Housing First, person-centered, harm reduction framework
Patience, creativity, flexibility, compassion, and sensitivity to persons with disabilities and other minority populations
Commitment to Pathways mission of ending homelessness and providing excellent mental health alternatives
Strong verbal and written communication skills
Comfort with rapid growth, thinking out of the box and organizational change
Excellent interpersonal skills - the ability to network with area service providers, funders, and community stakeholders.

Personal qualities of integrity, credibility, and commitment to the mission
Ability to travel statewide
Personal experience of homelessness, mental health challenges, incarceration, substance use or other life challenges considered an asset

Physical Requirements:
There may be the option for this job to be fully remote/virtual
If local: occasional driving to visit teams or participants; visual acuity to operate a motor vehicle required.
Extending and/or grasping with hand(s) and arm(s) in any direction at times when collecting paperwork, assisting with applications, completing hand-written forms, etc.
Ability to ascend and descend stairs occasionally required (in instances when businesses and/or meeting places do not have an elevator)
Regular operation of a laptop computer and other office machinery including frequent typing and extensive reading.
Frequent oral communication through spoken word for sharing of ideas and giving or receiving detailed information.
Compensation:
$105 - $115 per hour
$3000 Hiring Bonus Available
*Hiring bonus will be paid in the first pay cycle after the successful completion of the 60 day introductory period
