Job Title: Client Intake and Administrative Coordinator (LPN Preferred)

Job Summary:
The Client Intake and Administrative Coordinator will manage all aspects of client admissions, from the initial contact to the coordination of services, and will undertake various administrative responsibilities. An LPN is preferred for this role to leverage clinical knowledge in assessing client needs and facilitating care coordination. Candidates with similar healthcare experience are also encouraged to apply.
Key Responsibilities:
- Serve as the primary contact for new clients and their families, explaining services and guiding them through the intake process.
- Perform initial assessments of client health and care needs, ideally utilizing LPN knowledge for clinical insight.
- Liaise with healthcare providers, insurance companies, and other stakeholders to compile required documentation and approvals.
- Accurately maintain client records in the electronic health records (EHR) system, ensuring data integrity and compliance.
- Support the care coordination team in developing and implementing appropriate care plans for new clients.
- Manage and perform assigned administrative tasks, including scheduling, handling correspondence, and record-keeping.
- Uphold strict confidentiality in line with HIPAA regulations and other healthcare compliance standards.
- Offer continuous support to clients and their families, ensuring satisfaction with services and addressing any concerns.
Qualifications:
- Licensed Practical Nurse (LPN) license preferred; candidates with a high school diploma and significant experience in healthcare or client intake roles will also be considered.
- Minimum of 1 year of experience in a healthcare setting, with a strong preference for home health care experience.
- Proficiency in administrative tasks with excellent organizational and multitasking skills.
- Compassionate communication style with the ability to engage effectively with clients, families, and healthcare professionals.
- Proficiency in Microsoft Office and experience with EHR systems.
- Knowledge of healthcare services, insurance verification processes, and regulatory compliance requirements.
