HSO has been a breath of fresh air since the moment I arrived. I have never worked for an organization that values their employees so much! Instead of feeling replaceable, and like you are never "doing enough", I am thanked for my work often. Staff is very familiar with "upper management" and they usually know us all by name! - Experience with quality reporting preferred
- Provide telephone outreach and in-person consultation to members identified as high risk
- Assist in identifying gaps in care
- Collaborate with healthcare team to optimize patient adherence to medical plan of care
- Proactively outreach to members newly diagnosed with a targeted chronic condition
- Educate patient and family members about preventative screenings and identify barriers to care
- Interact with healthcare team
