Direct Care Staff

We are a local Pittsburgh company that provides care for adults with intellectual disabilities in each home where we have 24/7 staff to ensure safety, supervision, community involvement, assistance with improving independence, daily living skills, medication, socialization, etc.

We are now seeking reliable, responsible, energetic and compassionate full-time, part-time and PRN (fill-in) staff to join our growing team!

*Direct experience preferred.

Direct Care Staff Tasks (Details Below)

All responsibilities and tasks listed below are in addition to, but not limited to those listed in the regulatory job descriptions. This position is primarily responsible for ensuring the health and safety of assigned Clients and all Heart II Heart Clients. In addition, Direct Care staff is responsible to assist Clients to live life more fully and to increase their ability to carry out activities of daily living, as independently as possible.

Duties include, but are not limited to: Reports Directly to Staff Supervisor and Evening/ Weekend Coordinator

DAILY

• Complete Shift TURNOVER as assigned

• Review all Logs for completion and complete during shift

• Identify and report any staffing issues to Staff Supervisor

• Coordinate, delegate and Complete basic daily chores/shift tasks

• Report to Staff Supervisor and Document all staffing changes on schedule

• Report ALL compromises to the health and safety of Clients, Staff or H2H

• Ensure Daily Schedule and Recreation/Activity Schedule are followed

• Submit handwritten Incident Reports and notify supervisor

• Keep area neat and safe

• Ensure all Visitors show ID, explain purpose of visit and sign in

• Always Ensure to provide supervision unless WAIVER OF SUPERVISION has been signed and Staff Supervisor has provided the OK

• Complete delegated tasks from Management team

• Familiarize self with service definition, tasks and roles

• Familiarize self with emergency exit plan/emergency supplies

WEEKLY

• Ensure client clothes, linen and house linen are washed at least 1x per week MINIMUM

• Ensure a FULL house cleaning on WEEKEND every week

• Complete Weekly Medication Counts as required

OTHER/AS NEEDED

• Delegate shift task to ensure; (clothes washed, house cleaned, etc.)

• Report needed house repairs/supplies to On-Call/Supervisor

• Assist at other sites when necessary

• Other duties as delegated by Supervisors/Managers or CEO

• Complete Client personal inventory every 6 months
