Here at Dynamic we have immediate openings for a professional, reliable, and caring Medical Social Worker to join our Hospice team! Dynamic is a family owned, Medicare-certified team here to provide Hospice and Home Health visits to our patients across the Greater Los Angeles area.
Job Responsibilities include
- Provide Helpful Information to Patients and Family
- Help Arrange Services from Community Organization
- Assistance with Living Arrangements
- Crisis Management
- Guidance with Financial Problems
Qualifications
- Valid Licenses
- 1+ year of Home Health or Hospice experience preferred
