Aspire365 is a 12-month in-home and telehealth treatment program dedicated to addressing patients with both substance use disorders and primary mental conditions.

Aspire365 is actively seeking a Psychiatric Nurse Practitioner (PMH-NP) to join our dynamic multidisciplinary team, dedicated to providing compassionate care to individuals and families. This is a contracted part-time position, with the potential for expanded hours, focusing on delivering services through telemedicine visits.

Responsibilities include, but are not limited to:

Psychiatric assessments
Services for youth and adults 12 and older
Medication management
Ordering and interpreting lab work
Documents patient progress in Electronic Health Records (EHR)
Communicates and coordinates effectively with multidisciplinary clinical teams
Attendance in treatment team meetings as needed
Demonstrates core values of Aspire365
Periodic meetings with assistant medical director
