Licensed Practical Nurse (LPN) - Behavioral Health - Home visits 8-week contract

Are you an ambitious Licensed Practical Nurse with behavioral health interests?

We are seeking a dedicated and compassionate Licensed Practical Nurse (LPN) to join our Behavioral Health team. This unique position involves working closely with developmentally disabled individuals, particularly children and adolescents, to foster positive behavioral development through different interventions. This LPN will play crucial role in creating a therapeutic and supportive environment that facilitates emotional well-being and enhances coping mechanisms.

Your Responsibilities are:

• Assessment and Planning

Working Conditions:

• Desire to work with people who have developmental disabilities.
• Ability to travel within a 60-mile radius.
• The company will invest time and resources to get your NPI and Medicaid provider ID

Location: Cuyahoga County (east side or west side)

Time Commitment: 8-week contract (Monday to Friday - 8 AM to 4 PM)

• Our client is willing to train the candidate and pay for all of the training
• May work directly with the client after 8 weeks of contract

LPN Job Responsibilities

• Perform all skill, treatments, and procedures to the scope of practice as defined in the Nurse Practice Act - Ohio Revised Code (ORC) of clinical practice
• Implement treatment and plan of care, using sound judgement and critical thinking
• Maintain accurate, detailed, and timely documentation consistent with the plan of care and the nursing processes
• Document observed changes in patient's condition, situation, or care environment, and communicate to the Director of Nursing
• Adhere to the policies, procedures, and protocols of the agency of employment

Job Requirements

• Unencumbered license as in the State of Ohio
• Minimum one (1) year of documented clinical experience. Private duty, home care assistance and/or direct care experience at an assisted living or skilled nursing facility, etc., is preferred
• Strong clinical skills. Self-directing with the ability to work with little direct supervision
• Certified CPR and negative TB skin test or chest x-ray
• Licensed driver with reliable and insured automobile (proof of insurance required)
• Self-directing with the ability to work with little direct supervision
• Adhere to HIPAA and ensure confidentiality and security of the client's medical information
• Ability to read, write, speak, and understand English and communicate effectively
• Must be able to stand walk, squat, bend, kneel, reach, twist, push and pull. Varied assistance is required depending on the mobility of the client
• Must be able to lift up to 50 pounds
• Up-to-date vaccinations
