Certified Wound Care Specialist

Responsibilities
• Provides direct and indirect management of patients with wounds, ostomies, and incontinence by serving as both a clinician and consultant in the patient’s homes
• Provides leadership and direction in the coordination of all aspects of the WOC program as well as development of relevant policies, procedures, guidelines and standards of care that reflect Revival Home Care Agency’s mission, core values and standards of care
• Assesses patients to determine appropriate wound, ostomy and continence treatment plans • Collaborates with primary care physicians, surgeons, wound clinics, patients, families and other clinicians on WOC care
• Maintains and utilizes current knowledge of wound management theory, evidenced-based practice and new WOC products in the provision of care to patients with wounds, ostomies and continence care needs
• Evaluates, tracks and trends treatments, patient progress, effectiveness and makes recommendations for changes to the plan of care to achieve optimal outcomes in collaboration with patient, physician and case managers
Qualifications
• RN license in Maryland
• WOCN Certification
• 3 years wound experience
