Job Description

Job Title:   Diabetes Care and Education Specialist – Level 1

Department:  Ivy Center for Family Wellness

Reports To: Ivy Center for Family Wellness Director

FLSA Status:  Non-Exempt

Job Summary:

Coordinate and provide a full range of nutritional care services across the lifespan that promote health and supports patients in achieving health care goals. Assist the Ivy Center for Family Wellness in providing culturally appropriate counseling/information for the low-income, underserved patients of St. Vincent de Paul that struggle with, or are at risk for chronic diseases such as obesity, pre-diabetes, diabetes, and associated co-morbidities.  

Specific Duties:

  • Develops and implements Nutrition Care Process for patient care plan related to nutrition and chronic disease management. Correlates care plan with other health care professionals. Provides follow up for continuity of care. *Diabetes management care plans will be implemented under the supervision of a Certified Diabetes Care and Education Specialist. 
  • Ensure complete documentation of services rendered through Electronic Health Record for patient visits as outlined in policies and procedures.
  • Initiate referrals when indicated or outside of scope of practice to appropriate providers and/or programs. 
  • Act as education program coordinator as assigned by ICFW Director. 
  • Develop culturally-specific and evidence-based prevention education programs to prevent chronic disease and promote health, targeting particular vulnerable and at-risk groups of people. 
  • Facilitate services through individual interaction, groups session, in-person (on-campus and off-campus locations) and/or through distance learning.  
  • Identify and develop educational materials appropriate for age, literacy level, cultural background, and physical and cognitive abilities of recipients. 
  • Participate in the development of funding proposals and outcomes reporting.
  • Participate in the evaluation of the program using clinical practice guidelines.
  • Comply with evidenced based national care standards for chronic disease management.
  • Train and empower Community Health Educators, volunteers and interns in ICFW service delivery with information that is current, accurate, and evidence based. 
  • Work with community stakeholders and ICFW staff to achieve ICFW program goals and continuous quality improvement.
  • Establish and participate in outreach and promotion activities to expand the reach of services and increase the enrollment rate into educational offerings. 
  • Specific duties are not limited to those listed. Must perform other duties as directed by the ICFW Director. 
  • Complete the required organizational training, refresher or new skills training, and training offered by the ICFW. 
  • Comply with all applicable laws and regulations, including those governing participant privacy and data security (e.g., the Health Insurance Portability and Accountability Act [HIPAA])
  • Participate in and serve as a model for SVdP’s Culture of Philanthropy, and ensure SVdP volunteers feel welcomed and appreciated * Volunteer engagement and appreciation, as well as Culture of Philanthropy participation are part of all job descriptions at SVdP, and a very important part of the organizational culture and values

Required Skills/Abilities: 

  • Ability to relate to low income, minority individuals.   Appreciation for diversity and culture.
  • Comprehensive teaching skills with diverse groups.
  • Demonstrate professional and respectful attitude with clients/participants and staff.
  • Effective listening, written and oral communication skills.  Strong public speaking and presentation ability.
  • Organizational skills, interpersonal skills, problem-solving abilities and collaborative leadership experiences.  Flexibility, resiliency and ability to operate with minimal guidance.
  • Basic skills in word, excel, access Microsoft programs.
  • Bilingual; Spanish and English.
  • Arizona driver’s license, proof of vehicle insurance, and personal transportation.  
  • Availability from Monday through Friday between AM and PM hours. Some availability may be required throughout the year on the weekends.

Education and Experience:

  • Bachelor’s degree in Nutrition (Dietetics).
  • Eligible to sit for the Examination for Registered Dietitian Nutritionists (RDN) credential as recognized by the Academy of Nutrition and Dietetics. 
  • Successfully pass the Examination for Registered Dietitian Nutritionists within 3 months of employment. Maintain credential through duration of employment.
  • 0-2 years of direct care experience in community work, 25% of time devoted to diabetes specialty practice. 
  • Proof of professional liability insurance, upon receipt of RDN credential (Annually).

Physical Requirements: 

  • Able to stand, walk, sit, bend, reach and lift up to 30 pounds are required for this position
