ABOUT THE ORGANIZATION:
Established in 1970, Boley Centers, Inc., is a private, non-profit organization serving individuals with mental disabilities, individuals and families who are homeless, Veterans and youth in Pinellas County. Boley Centers, Inc., a nationally recognized and accredited behavioral health care agency, has an opening for a full-time Licensed Practical Nurse.
FACT - Florida Assertive Community Treatment
FACT is a hybrid community-based program which sees clients in the community and in the office. FACT is an assertive treatment model providing intensive support to those experiencing severe and persistent mental illness in a multidisciplinary fashion with an emphasis on a team approach.
JOB SUMMARY:
This opportunity affords applicants to participate in their own scheduling, utilize tools to document from home, and especially build rapport with our clients. Our clients on average stay with our team for approximately 5 to 7 years, which provides a unique view at continuity of care and understanding the healthcare determinants of our population.
Our nurses will provide clinical assessments of health needs, mental health needs, case management, and general health education to clients. They will also collaborate with physician offices, our onsite psychiatric NP, the pharmacy, and other partners in care.
Our nurses are provided the ability to help develop their role and are provided the creative freedom to provide the most excellent care to our clients.
EDUCATION AND EXPERIENCE:
High School diploma or GED and LPN licensure.
Two to three years’ experience working with adults with severe mental health disorders.
Knowledge of clinical/counseling techniques, verbal and physical crisis intervention and performing annual physical exams.
SKILLS AND ABILITIES:
Good skills and demonstrated abilities in oral and written communication techniques, coordination/cooperation, follow through, interpersonal relations, attention to detail, use of basic math/language, following verbal and written procedures/instructions, recording information orally and in writing, instructing, teaching, coaching, internal and external public relations, working independently, verbal/oral/physical crisis intervention techniques, problem solving, negotiating, flexibility, decision making, counseling/evaluation techniques, organizing own work, able to work in a multidisciplinary team and such others that are usually and customarily considered necessary for satisfactory completion of essential duties.
SPECIAL REQUIREMENTS:
Valid Florida driver's license, auto insurance and own transportation vehicle required.
