Licensed Practical Nurse (LPN)

The Center for Wound Healing & Hyperbaric Medicine (CFWH) provides management and consulting services to healthcare providers in the area of wound care and hyperbaric medicine. By focusing on the needs of the patients as well as the partners we serve, The Center for Wound Healing & Hyperbaric Medicine is able to bring advanced wound healing strategies to many communities around the country. This position is located in Englewood, CO.

Job Overview at Center for Wound Healing & Hyperbaric Medicine LLC:

Under the supervision of the Program Director and Nurse Case Manager, the LPN will provide general clinical care to patients afflicted with disorders of the integumentary system including wounds issues. This position involves case management and involves administrative duties about 50% of the time. The LPN is responsible to adhere to and enforce the nursing process within the Center for Wound Healing & Hyperbaric Medicine and the continuum of care. The nursing process is utilized to assess, diagnose, establish a plan with outcome criteria, intervene, evaluate, and document human responses to actual or potential health problems. This practice includes dependent, interdependent and independent functions. Acts within the guidelines of federal and state laws, Joint Commission or DNV ISO 9001 Standards, and professional ethical practices.

Essential Responsibilities:

  • Conducts nursing assessments and thoroughly documents in electronic medical record (EMR) health status by collecting objective and subjective data from observations, exams, interviews and written records; sorting, selecting, reporting and recording the data; validating, refining, and modifying the data. Bases the scope and depth of assessment on the patient’s immediate condition or need.
  • In collaboration with the patient and Physician, develops a plan of care based on nursing assessment and diagnosis that prescribes interventions to attain expected outcomes. This includes identifying the holistic needs of the patient, treatment decisions and priorities. Identifies community resources needed for continued care and makes referrals.
  • Implements the plan of care and the nursing interventions for the patient by electronically executing nursing orders, giving direct care, assisting with care, determining the responsibilities that can properly and safely be assigned or delegated, providing an environment conducive to safety and health, communicating interventions and responses to other members of the health care team and executing the regimen prescribed by a licensed practitioner.
  • Explains plan of care to the patient in a way that is understandable and educates the patient thoroughly. In additional to explaining the plan of care, provides educational materials to the patient.
  • Evaluates the responses to nursing interventions by documenting in the electronic medical record (EMR) and communicating evaluation data to appropriate members of the health care team, utilizing data as a basis for reassessing client health status, measuring outcomes and goal attainment, modifying nursing diagnoses, revising strategies of care and prescribing changes in nursing interventions.
  • Thoroughly documents in electronic medical record (EMR) patient information in an accurate and timely manner, according to organizational policies, procedures and guidelines.
  • Demonstrates and applies continuous quality improvement knowledge and skills.
  • Verbalizes and demonstrates knowledge of procedures for maintaining security, confidentiality and integrity of employee, patient, family and other medical information.
  • Demonstrates skill in use of equipment relevant to position including modern technology; verbalizes knowledge of appropriate safety procedures.
  • Functions as an expert clinician in assessing, planning, intervening and evaluating care for the patient with wound and skin disorders.
  • Establishes protocols and processes of care on a case by case basis that result in optimal care and clinical outcomes for patients while maximizing the use of resources.
  • Demonstrates leadership in the development, implementation and evaluation of patient education programs.
  • Participates in committees and/or projects as assigned.
  • Supports Nurse Case Manager with staff competencies at 90 days, annually, and as required.
  • Support the Nurse Case Manger’s efforts to manage clinical staff and enforce policies and procedures including daily charting in electronic medical record (EMR).
  • Supervise, train and educate Wound Care Technicians, delegate appropriate tasks within scope of practice.
  • Ensures staff adheres to company, hospital, and accrediting body clinical standards.
  • Regularly audits Electronic Medical Record (EMR) for completion.
  • Other duties as assigned.
  • 25% Clinical, 25% Case Management, 50% Administrative

Required Qualifications for This Role Include:

  • Licensed Practical Nurse, graduated from an accredited school or college.
  • Current state LPN license.
  • 2+ years of clinical nursing experience, preferably hospital or outpatient clinic.
  • Experience in Wound Care required.
  • Current BLS from approved agency.
  • Strong verbal and written communication skills.
  • Proficient in Microsoft Office: Word, Excel, PowerPoint, Outlook.
  • Positive background check, negative drug screen, and hospital credentials (as needed) required.
  • Proficient in Electronic Medical Record (EMR) documentation

Travel:

  • This position may require light ground and/or air travel.

Physical Demands:

  • The physical demands described are representative of those that must be met by a Licensed Practical Nurse (LPN) to successfully perform the essential competencies, duties, and requirements of this position. These include standing, walking, driving, lifting up to 50 pounds, sitting for extended periods of time and looking at a computer screen for extended periods of time. In addition, pushing, pulling, stooping, squatting and shifting heavy objects, as well as air travel, may be required. Employees should ask for assistance if needed.
