Care Coordinator

Job Summary:
Griswold Home Care of Camden County is seeking a highly motivated and compassionate individual for the role of Care Coordinator. This is a full-time administrative position in the healthcare industry located in Berlin, New Jersey. The ideal candidate will be responsible for coordinating care services for our clients and will serve as a liaison between clients, caregivers, and other healthcare professionals. This is an individual contributor role in which the Care Coordinator will work closely with our team to ensure high-quality care for our clients.

Compensation & Benefits:
The compensation for this position is $42,000 per year, paid biweekly. In addition, the Care Coordinator will be eligible for benefits including health insurance, paid time off, and retirement contributions.

Responsibilities:
•Coordinate and manage care services for clients, including scheduling and staffing of caregivers.
•Serve as a liaison between clients, caregivers, and other healthcare professionals to ensure proper communication and coordination of care.
•Conduct initial client assessments and develop individualized care plans.
•Monitor and track client progress, making necessary adjustments to care plans as needed.
•Maintain accurate and up-to-date client records and documentation.
•Keep clients and their families informed and involved in the care process.
•Provide ongoing support and guidance to caregivers.
•Conduct regular quality assurance checks to ensure high-quality care and client satisfaction.
•Comply with all state and federal regulations, as well as company policies and procedures.
