We are seeking a Recovery Model (of mental health) oriented LCSW, MSW, LMFT, MFT, or LPCC to join our team! You should be licensed, and if not, you should have experience working with severe and persistent mental illness. You will provide in person therapy and clinical case management to address legal, social, medical, mental health, substance use, emotional and administrative support to members individually and in groups. You will provide clinical direction and support to residential staff for milieu therapy, case management, and coaching as a clinical lead. You will be working with individuals who have been arrested, but not convicted due to being incompetent to stand trial. They come from jail as a diversion from the State Hospital. They have often experienced chronic homelessness. We change lives and see remarkable recoveries.

This is the job for someone that loves direct client care and is creative. We do not have DMH billing requirements, therefore, the focus is on being creative in designing great programming and individualized treatment, effectuating change in the lives of our members, and managing your treatment team. We are a passionate team dedicated to giving the most vulnerable individuals a fresh start with the long-term support they need to be successful. Our members really do come first!

Duties

-Maintain a caseload of 16-20 members.

-Complete a comprehensive biopsychosocial assessment to identify and address member needs.

-Establish individualized interventions/treatment plans with your team to promote identified goals.

-Use the Recovery Model to instill hope, empowerment, self-responsibility and support members in developing meaningful roles in life.

-Provide individual and group psychotherapy, case management, individual and group rehabilitation, crisis intervention, conflict resolution, advocacy, psychoeducation, coordination of services and appropriate follow-up on all referrals.

-Use cognitive behavioral therapy, motivational interviewing, trauma informed care, motivational interviewing and other member centered therapeutic interventions.

-Encourage and promote self-reliance, providing coaching to build self-management skills. (Doing with vs. doing for)

-Review clinical notes routinely to ensure that member follow up is conducted as required.

-Utilize a team approach, through regular collaboration with multidisciplinary staff and facilitate members’ health care needs are sufficiently met.

-Provide clinical guidance to residential staff on milieu therapy, individual, and group rehabilitation serving as a clinical lead.

-Maintain required documentation in and update clients records in accordance with governmental. and agency policy.

-Participate in regular supervision, team meetings, case conferences and staff trainings.

-With the help of your multidisciplinary team, ensure your members and team go on fun community outings to build life skills.

- Role model hope, positivity, and teamwork to your team and members you serve.

-Maintain a flexible schedule. *Perform other duties as required.

Qualifications

-Master’s Degree in Social Work, Marriage and Family Therapy, Psychology, Counseling or other related field required. License is preferred, but not required.

-**Experience working collaboratively with people experiencing homeless, severe and persistent mental illness, substance use, and legal issues.

-Knowledge of non-violent crisis intervention, trauma informed care, motivational interviewing, harm reduction techniques.

-Must be proficient in the following computer applications: Microsoft Word, Excel, and Outlook.

-Bilingual (Spanish/English) desirable but not necessary.
