Summary
Our Qualified Mental Health Professional - CS (QMHP) community services follow a strengths-based model that help develop and build skills that help children and families function autonomously and successfully. The QMHP-CS will work with the clinical team in developing a Service/treatment Plan that is focused on the child and family needs and recognizing their strengths. QMHP-CS will provide mental health/behavior support services to families and children with behavioral issues, anger management, and severe emotional disturbances by engaging the child and family in weekly skills training services as well as Targeted Case Management when appropriate. QMHP-CS will assist the family in any Case Management (Routine/Intensive) activities that can be beneficial to the youth and family.
Qualifications:
- A minimum of a bachelor's degree from an accredited college or university with a minimum number of hours that is equivalent to a major in psychology, social work, medicine, nursing, rehabilitation, counseling, sociology, human growth and development, physician assistant, gerontology, special education, educational psychology, early childhood education, or early childhood intervention
General Responsibilities: The workers is responsible for providing medical necessary services to the client in accordance with the treatment plan that is designed to meet the mental health, educational, vocational, residential, financial, social and other needs of the consumer.
- Maintain a caseload of 10 clients, minimum 6
- Submit a minimum of 32 billable hours weekly
- Attend weekly supervisions and training sessions
- Serve as an intricate member of the SP team to ensure continuity of care and supports the services 24 hours a day, seven days per week as necessary
- Provides structured face-to-face scheduled sessions to client based on the treatment plans
- Provide services in the home, school, office and community setting
- Ensure that the majority of mental health services are primarily rendered in the community and ensure that limited services remain clinic based.
- Documentation completed within 72 hours of service
- Work collaboratively with and provide ongoing updates to the clinical team including psychiatrist as needed
- Completes and attest to completion of all mandatory training as identified in TRR
- Ability to work efficiently and effectively both individually and as part of a team
- Ability to appropriately accept feedback through the supervision process - thus displaying the willingness to learn, grow, and improve
- Well organized and adaptable
- Demonstrates good insight and judgment
- Effective conflict resolution skills
- Provide skill building and case management services to clients
- Submit reports timely
- Effective oral and written communication skills
- Ability to work with children and family to deliver skills through approved curriculum
- Ability to support the agency's culture, growth, and success through communication, accountability, and positivity.
- Competent in using Microsoft Word, Excel, Power Point, Zoom, the Internet, and other software applications
