Basic Purpose Of The Job
Under the direction of the CMO and EVP of Clinical Services, the clinical Social Worker provides direct service to clients and their families. This includes counseling and seeking resources to answer unmet needs. The CSW also provides consultation services and in-service education programs related to the psychosocial aspects of the health care to other professional staff members on the healthcare team.
Follows infection prevention and control procedures.

Job Requirements
Minimum Education
- A Bachelor’s or Master’s degree in social casework from an accredited graduate school of social work, as define by the council on social work education preferred
Minimum Work Experience
- Complete a clinical practicum to include crisis intervention and brief solution-focused therapy. Knowledge of community resources required
Required Licenses/Certifications
- CPR

Essential Functions

- Upon assessing patient needs, social worker makes all appropriate referrals to other community service agencies in a timely manner, evidence by documentation
- During each visit, the plan of care is reassessed to meet client and family/caregiver needs and ensure quality of care, as evidenced by documentation in the clinical record.
- During each visit demonstrates the skills and judgment necessary to implement the plan of care, interventions and procedures necessary for the care of the client, as evidenced by the chart review and with care team input
- Is knowledgeable of and utilizes appropriate community resources.
- Maintains standards for licensing in the social work field.
- Attends all required safety training programs and can describe his/her responsibilities related to general safety, department/service safety, and specific job-related hazards
- Follows the health center exposure control plans/bloodborne and airborne pathogens.
