Pay: $22.07- $26.66 /hour (based on experience & education)

Location: Salem, OR

Schedule:  Monday - Friday (8:30am - 5pm)

General Position Summary:

The Personal Agent provides trauma-informed case management services to persons with intellectual and/or developmental disabilities, using person-centered practices to assist individuals in accessing needed medical, employment, social, educational, and other services. 

Minimum Requirements:

1. Bachelor's degree in behavioral science, social science, or a closely related field OR a bachelor’s degree in any field and one year of human services related experience OR an associate’s degree in a behavioral science, social science, or a closely related field and two- three years of human services related experience.

2. Knowledge of the public service system for developmental disabilities services in Oregon. 

3. Experience working in a team setting. 

4. Reliable transportation, valid Oregon driver’s license (or the ability to obtain a valid Oregon driver’s license within 30 days of hire) with a driving record that permits coverage under the agency’s corporate auto liability policy and personal automobile liability insurance. 

5. Previous experience in Person Centered Planning and Self-Determination preferred. 

6. Complete required case management core competency modules: (i) Tier 1 trainings within 30 days of the employment start date and before working unassisted, (ii) Tier 2 trainings within 90 days of the employment start date. 

7. Attend or participate in ISP training that is endorsed or sponsored by the Department within first year of position. 8. Complete 20 hours per year of Department sponsored training or other training in the areas of intellectual or developmental disabilities.

2. Knowledgeable in case management principles, procedures, and practices. 

3. Knowledge of Methods, procedures and techniques used in the development of Individual Support Plans 

4. Knowledge of trauma-informed service delivery and sanctuary practices. 

5. Knowledge of Quality Assurance Principles. 

6. Knowledge in health and human service systems 

7. Knowledge of the community, public and local agencies 

8. Knowledge of consumer rights. 

9. Ability to advocate and assist individuals in maximizing their abilities for self-determination through informed choice. 

10. Ability to maintain professional relationships. 

11. Ability to present ideas effectively, in both oral and written form 

12. Ability to identify and develop community resources. 

13. Ability to work collaboratively with other local and community agencies. 

14. Ability to present to large and small groups. 

15. Ability to use computer applications or other automated systems such as spreadsheets, word-processing, calendar, e-mail and database software in performing work assignments. 

16. Ability to assess, plan, organize and follow-up with projects or plans. 

17. Ability to comprehend and follow oral and / or written directions. 

18. Ability to Work independently and within diverse work teams. 

19. Ability to handle complex situations and to address conflict. 

20. Ability to maintain confidentiality.

21. Ability to maintain case management records, document case files and information within required/mandated timelines.

Essential Position Functions and Key Work Processes:

1. Assist persons with developmental disabilities to plan, implement, and monitor their services. 

2. Recognize personal strengths, to connect people with natural supports, resources, and professional supports. 

3. Develop and periodically revise ISP or Annual Plan based on the information collected through an assessment or reassessment that specifies the desired outcomes, goals, and actions to address the medical, employment, social, educational, and other services needed by the eligible individual. 

4. Assist customer to access available services, including referral and related activities. 

5. Monitor services, including activities and contacts that are necessary to ensure that the ISP or Annual Plan is effectively implemented and adequately addresses the needs of the eligible individual. 

6. Participate in the delivery of protective services for adults when required by Oregon Administrative Rule or Oregon Revised Statute. 

7. Provide information, education, and technical assistance including but not limited to individual rights, choice advising, reestablishment of eligibility, transition planning and complaint resolution. 

8. Authorizing services in the Department’s electronic payments and reporting system 

9. Complete all required documentation in individual case files in a timely manner. 

10. Provide support to Friendship Connections, meeting community needs, to include developing neighborhood resources, analyzing system needs, coordinating new resources, and resolving access problems. 

11. Develop and maintain relationships with other agency personnel, community leaders, and neighborhood Friendship Connections members. 

12. Coordinate training activity that supports individuals and families and promotes a sense of community. 

13. Maintain up-to-date claims and customer notes on a weekly basis. Adhere to the targeted claims expectations. 

14. Maintain and share with co-workers’ business calendar to include customer appointments, team meetings, community event attendance, and time away from the office.

Secondary Position Functions:

1. Assist Brokerage Leadership Team with projects, programs and meetings as required. 

2. Work as a collaborative team member to support and back up other Personal Agents as needed.

3. Participate on at least one committee within the organization or outside of the organization that has a direct 

impact on operations and services.

4. Participate in professional development opportunities.

5. Other job duties as assigned.

6. Support multiculturalism by treating all people with dignity and respect, not engaging in any discriminatory behavior, participating in four hours of diversity training each calendar year, and support the program’s cultural competency goals. 

7. Act as a team member including, but not limited to active participation, working well with others, and supporting team efforts and goals. 

8. Use a solution focused/problem solving approach when conducting agency business. 

9. Provide and maintain professional, respectful communication with all persons while representing ISN.
