Care Navigator-Supervisor (Telehealth)

Excelsior Wellness Center is for a Team Supervisor within our VIEW program.

As a VIEW Team Supervisor within VIEW program at Excelsior Wellness, you play a crucial role in our mission to promote mental, emotional, and physical well-being in the broad community of Washington State. You facilitate connections between community providers where a client is currently at and where they need to go in their journey to wellness by holding a caseload as a Care Navigator. Your role is to provide connections, direct service, support, and evaluation to individuals who may be experiencing mental health or substance use issues. Additionally, you provide direct supervision for Care Navigators, Youth Peers, and Family Peers.

Your daily interactions with our clients make a significant impact by offering them hope, connection, and the knowledge that recovery is possible. Your role requires active listening, compassion, and the ability to foster a non-judgmental, supportive environment.

Your daily support for VIEW Peers and Navigators help provide the clarity, confidence, training, and competence to excel in their roles.

You collaborate closely with our clinical / counseling teams and community providers across Washington State, and ensuring clients are being connected to where they need to go. Your role is instrumental in helping our clients regain control of their lives and work towards their wellness goals.

How to Qualify:

• High School Diploma required. BA in counseling or related field preferred.
• Ability to be licensed through the Washington State Department of Health as an agency-affiliated counselor.
• Comprehensive knowledge of public systems and the ability to obtain and organize information for resources in various areas across Washington State.
• Completion of recognized Peer Support Specialist training program or certificate preferred.
• Prior supervision experience, is preferred.
• Experience in Behavioral Health Case Management preferred.

• Ability to communicate effectively and advocate for client, staff, and programmatic needs to leadership and staff.
• Strong organization, communication, time management, and active listening skills.
• Knowledge / elevated skills in computer applications, such as Word, Excel, and Teams.
• Empathy and the ability to build trust with clients.
• Ability to organize information from conversations to the creation of task lists and prioritizing tasks based on level of need.
• Ability to work effectively in a team and collaborate with clinical and counseling professionals.

• Ability to pass a criminal background check, per organizational standards.
• Must be 21 years or older.

Core Responsibilities:

• Provide Care Navigation and a safe, non-judgmental environment for clients.
• Collaborate with the clinical and counseling teams to enhance client care and a provider and a supervisor.
• Make referrals to internal and external services.
• Complete CANS evaluations as needed.
• Ensure daily responsibilities of assigned staff are being completed
• Participate in team meetings to share insights and updates.
• Review client progress and provide ongoing support.
• Track client service planning and long term goals.
• Assist in the development of recovery-focused initiatives and programs.
• Contribute to the evaluation and improvement of VIEW services
