Salary: $25.80 - $30.70, DOE.
Your Role:
As an Enhanced Care Coordination Specialist, you'll provide community based, high-touch case management services to individuals who are high utilizers of Chapa-De Indian Health. Your mission is to provide patient-centered care which will include collaborating with healthcare providers, advocating for patients, and connecting them with the resources they need to thrive.

Your Role:
As an Enhanced Care Coordination Specialist, you'll provide community based, high-touch case management services to individuals who are high utilizers of Chapa-De Indian Health. Responsibilities:
- Develop and implement care plans tailored to individual patient needs
- Coordinate appointments, referrals, and follow-up care
- Serve as a liaison between patients, healthcare providers, and community resources
- Evaluate the effectiveness of care plans and make adjustments as needed
Experience and Skills
Requirements:
- Bachelor's degree in social work or related field; or equivalent combination of education and experience
- Min 2 years experience in care coordination or case management
- Strong communication and interpersonal skills
- Ability to multitask and thrive in a fast-paced environment
