ABOUT OREGON INTEGRATED HEALTH

Oregon Integrated Health is a Tier 4 Patient Centered Primary Care Medical Home. We have clinic locations in Portland, Eugene, Salem and Florence, Oregon. Our clinics are integrated clinical models with Primary Care Providers that include Naturopathic Physicians, Nurse Practitioners and Medical Doctors, Psychiatric Mental Health Nurse Practitioners, Counselors, Chiropractors, Acupuncture, Physical Therapy, Massage Therapy and Eugene has an Expanded Practice Dental Hygienist for our Oral Health program.

Florence currently has one Psychiatric Mental Health Nurse Practitioner and Acupuncturist. Salem recently opened earlier this month currently has one Family Nurse Practitioner.

The EUGENE clinic is looking to fill one Psychiatric Mental Health Nurse Practitioner (PMHNP) position - Full or Part Time. All of our clinic locations utilize telemedicine extensively with our patients and we will continue to do so especially during the COVID-19 outbreak.

JOB DUTIES AND RESPONSIBILITIES:

  • The Mental Health Nurse Practitioner will assess and provide care in an integrated setting along with other primary care, behavioral and mental health and alternative care providers.
  • Medically evaluates patients and assesses their medical status.
  • Diagnoses the mental status of patients of Oregon Integrated Health by mental assessment and physical examination.
  • Determines, establishes and administers an overall treatment program for patients with a persistent or serious mental illness.
  • Determines the clinical need for medications, and prescribes medications as part of a treatment program.
  • Acts as the clinical expert on psychiatry and provides information, consultation and assistance to other health care professionals at Oregon Integrated Health.
  • Coordinates care with private physicians in the community as needed.
  • All providers have access to Medical Support staff as well a Patient Health Navigators for patients with complex medical and behavioral/mental health disorders.
  • New patient visits are 60 minutes, Return patient visits are 30 minutes and urgent medication management appointments are 15 minutes.
  • Refer patients to other health professionals in the treatment of diseases requiring further diagnosis and evaluation.
  • Provides appropriate medical record documentation in an EHR on all patients in accordance with clinic policy.
  • Works with telemedicine for patients that maybe in Portland and Eugene as needed.


QUALIFICATIONS, SKILLS AND ABILITIES
  • Previous experience in a Private practice or Family Care medical setting
  • State of Oregon license active
  • Master's degree in Nursing from an accredited college or university, and completion of a Nurse Practitioner program specific to the expanded specialty of Psychiatric/Mental Health Nurse Practitioner (PMHNP).
  • DEA active without restriction
  • National Provider Identifier (NPI)
