Job Summary: Physical Therapist Care services provide skilled therapy intervention and treatment to patients in rehabilitation centers. Including appropriate utilization of treatment plans and assessment skills. Monitor the patient's status, progress toward goals, and compliance with physician's orders. Responsible for educating patients and caregivers while preparing the patient/caregiver for discharge as appropriate.

Physical Therapist Responsibilities

• Provide skilled Physical Therapy with the physician's plan of treatment.
• Assist the physician in evaluating and assessing the client's level of functioning by applying appropriate tests and assisting in developing and revising the care plan.
• Treat clients to relieve pain, develop and restore function, and maintain maximum performance using appropriate physical therapy treatments, modalities, and procedures as stated in the physical therapy plan of care.
• Observe, record, and report the client's reaction to treatment and any changes in the client's condition to the physician, Clinical Man, ager, and Case Manager.
• Educate the client/caregiver and health care team in the phases of physical therapy. As appropriate, they may assist the client in using prosthetic and orthotic devices and durable medical equipment.
• Provide staff with n-service education as directed by the Clinical manager
• Assess and regularly reevaluate the needs of the client. Develops, implements, and revises the client's plan of care.
• Initiate diagnostic, preventive,tive, and rehabilitative procedures appropriate to the client's safety.
• Assisted Wa with a plan of care and maintained continuity and multidisciplinary clients whenever needed to ensure good care coordination.
• From care coordination through interdisciplinary case conferences to communication with physicians and field staff.
• Develop, prep, and maintain individualized client care progress records accurately, timely, and according to agency policy and procedures.
• Participate in the agency's quality improvement program.
• Keep abreast of physical trends and knowledge for service provision, documents, time, and care coordination.
• Participate in staff meetings, in-service programs, MS, and Utilization reviews.
• Supervise the Physical Therapy Aide at least every 30 days.

Education:

• Bachelor's (Required)

Experience:

• Physical Therapist: 1 year (Required)

License/Certification:

• Physical Therapy License (Required)
• CPR Certification (Required)
