Job Description

Elaine Ellis Center of Health is hiring a PROFESSIONAL COUNSELOR to provide counseling services in our DC and Maryland locations. The ideal candidate for this position is licensed in both DC and MD, is able to provide counseling services to outpatients for mental health and substance use disorders, and can work collaboratively with the mental health team. EECH offers a 4 day work week, competitive salary and benefits and a team oriented environment. 

 

SPECIFIC DUTIES AND RESPONSIBILITIES:

·         Conduct and document initial assessment of patient and treatment program readiness.

·         Conduct and document on-going assessments of patient’s strengths, counseling needs and progress.

·         Develop appropriate treatment plans and goals based on individual history and needs.

·         Ensure an appropriate safety plan is established.

·         Review and analyze treatment plans to evaluate progress.

·         Collaborate with prescribing physicians and other health professionals regarding patient’s treatment and progress.

·         Support patients, families and caregivers in self management, self efficacy and lifestyle and behavioral changes.

·         Maintain a therapeutic relationship with patients to achieve desired goals.

·         Abide by laws, regulations and standards of practice regarding ethics for the provision of treatment under the scope of license and certification.

·         Develop aftercare programs and follow-up procedures when treatment is completed.

·         Record and maintain patient information accurately and timely.

·         Provide education to patients and community members.

·         Refer patients to other providers and specialists as appropriate.

·         Consult with and provide consulting services to other providers.

·         Participate in provider meetings and other Center meetings as needed.

·         Adhere to the Uniform Data System (UDS) productivity standards. 

·         Participate in outreach and offsite programs as assigned by the CMO.

·         Attend and participate in teaching and general staff conferences.

·         Actively participate in team training for patient centered care coordination.

·         Continually update education, therapeutic techniques and methodology; apply new knowledge to your position.

·         Perform other duties and assist with special projects as assigned.
