Nurse Practitioner - Wound ($250/hr)

Immediate opening for a self-motivated and independent Part-Time Per Diem NP to join our growing team! Steward Home Health Agency, Inc. is an established home health agency looking to partner with a Wound Specialist. NP should have knowledge in Advanced wound care, ostomy care, and skin grafting in long-term care facilities and patient residences across Los Angeles County.

Responsibilities:

• Travel and treat our homebound patients
• Conduct the initial evaluation and all follow-up visits
• Ability to perform complete physical exams
• Perform wound care procedures, including debridement
• Prescribe medications and dressings as needed
• Perform skin grafts when medically necessary
• Initiate referral for additional services as needed
• Document full scope of care from start of care to discharge
• Provide patient support and education
• Participate in continued training activities
• Recommend and/or order medical supplies for treatment

Qualifications:

• NP must be a graduate of an accredited Nurse Practitioner
• License to practice Medicine as an NP with a current valid state license.
• Must be Board Certified and able to provide proof of Board Certification.
• Must have prior Wound Care experience and/or Wound care Certification.
• Must be wiling and able to travel in Los Angeles County.
• Patient-centric approach
• Excellent verbal and written communication skills in a professional environment
• Must be proficient with medical instruments and equipment.
• Ability to communicate effectively, in verbal and written form.
• Sound understanding of all federal and state regulations.
• Ability to lift 50 lbs. or more.
• Must have a full range of mobility.
• Must be able to reposition patients.
• Valid and unrestricted driver’s license
