POSITION SUMMARY:

The Specialized Community Health Worker will provide direct assistance, navigation, outreach, education (one-on-one and group), and case management to existing health center patients and community members throughout the PCHP service area related to HIV prevention and education.


RESPONSIBILITIES:

· Upon hire, successfully complete comprehensive training to best serve high-risk populations, including but not limited to: Community Health Worker (CHW) certification, Rapid HIV/HCV Tester Certification & Case Management for HIV, and Patient Centered Medical Home (PCMH).

· Assist the Director of HIV Prevention & Education to ensure that relevant program data is collected, tracked, and reported in the appropriate systems (including, but not limited to the electronic medical record, tracking logs, forms, etc.) within a timely manner. Ensure that quantitative and qualitative data is captured to show project impact throughout the duration of the project.

· Conduct public education & outreach activities to raise awareness about HIV prevention & education and to directly increase the number of individuals tested for HIV in the service area.

· Increase the number of individuals tested for HIV outside of the clinic setting, provide case management for patients prescribed PrEP, and link any newly diagnosed HIV clients to medical care (initially via defined partners) within 30 days of diagnosis.

· Manage, facilitate, and attend community events throughout the PCHP service area to provide HIV testing & linkage to care. Work closely with community partners throughout the service area to successfully meet outreach and testing goals.

· Must be willing and able to work non-traditional hours (evenings and weekends) and work outside of a traditional office setting at times (i.e. community events, etc.).

· Other duties, as assigned.


REPORTING RELATIONSHIPS:

The Specialized Community Health Worker reports to, is supervised by, and is evaluated by the Director of HIV Prevention & Education.


QUALIFICATIONS:

· High school diploma required. Associate degree or higher in public health, community health, or nursing preferred.

· Excellent written and verbal communication skills, with proficiency and experience in public speaking, group presentations, and delivering one-on-one education.

· Strong desire to serve as a community liaison for the health center- serving as a key point of contact for individuals in need of HIV testing, risk reduction, and linkage to care.

· Exhibit an ability and comfort in engaging patients and community members in candid conversations about sexual health and practices, sexual orientation, gender identity, risky behaviors, HIV testing, and infectious diseases.

· Must be self-motivated, and flow easily between independent work and within a team context.

· Must be able to follow instructions closely, meet grant goals (number of individuals tested, linked to care, etc.), and provide required documentation back to supervisor in a timely manner.

· Must have reliable transportation and travel up to 20% of time (within the service area).

· Must be proficient in Microsoft Office and have an understanding of electronic health records (eClinicalWorks preferred).


EVALUATION:

The evaluation of work performance is on-going and will be performed by the Director of HIV Prevention & Education. It will include the specific duties and responsibilities of this position description plus employee attitude and general working behavior. Formal evaluation will normally be performed annually but may be initiated at other times by the supervisor.


SALARY POSITION:

The salary for the Specialized Community Health Worker position is at Grade 4 of the community health center's salary scale.
