Central Intake Coordinator - LVN

Overview of Position

Under the direction and supervision of the Administrator, the Central Intake Coordinator – LVN is responsible for recording and accepting referrals, determining the level of care necessary, verifying insurance coverage and acting as a liaison with referral sources and third-party payers.

Core Performance Requirements

• Receives incoming telephone calls (referrals) from physicians, Discharge Planners and physicians’ office personnel regarding new patients.
• Demonstrates effective problem-solving, good judgment, and confidential handling of sensitive information.
• Communicates in a professional and courteous manner with patients/families, colleagues and supervisors.
• Ensures complete intake referral information, including face-to-face, diagnosis, PDGM specific, co-morbid profile, and hospice appropriate level of care.
• Must be able to multi-task and work in a fast pace, high energy environment.

Qualifications & Requirements

• California LVN licensed.
• Supervisory experience.
• Demonstrate ability to use computer programs.
• Knowledgeable regarding reimbursement mechanisms – Medicare, Medi-Cal and private pay.
• Demonstrate an interest and ability to work effectively with the community.
• Home Health and Hospice experience is preferred.
