JOB SUMMARY

The Licensed Vocational Nurse administer medications, takes and records patients' vital signs, reports adverse reactions to medication or treatment to medical personnel in charge. The Licensed Vocational Nurse assesses ongoing symptomatology and provides crisis intervention as needed. The Licensed Vocational Nurse assists clinical staff in implementing the Patient Assistance Program.

  • Assumes an active role in the treatment team, providing appropriate input as needed.
  • Completes all charting and documentation in the electronic health records as contractually required and in timely manner as dictated by program requirements.
  • Coordinates medication orders and refills with pharmacies.
  • Responds promptly to client calls regarding medication issues.
  • Provides client education on medications and their side effects.
  • Collaborates with Psychiatrist and Registered Nurse to monitor client wellbeing and medication compliance along with medication efficacy and side effects.
  • Takes and records vital signs.
  • Initiates appropriate and timely interventions on an ongoing basis and provides crisis intervention as needed.
  • Provides case management for an assigned group of clients including coordinating and monitoring the activities of the treatment team.
  • Assumes responsibility for developing, writing, implementing, and evaluating overall treatment goals.
  • Provides individual supportive counseling and symptom management.
  • Educates and support clients and family members, which can include individual and group services.
  • Conducts psychiatric history; mental status; medical and dental history.
  • Assists in administering Patient Assistance Program, ensuring efficiency and accuracy in all aspects.
  • Provides crisis call services after business hours.
  • Coordinates primary care as needed.
  • Initiates appropriate and timely interventions on an ongoing basis; provides crisis intervention as needed.
  • Comply with all MHS and program rules, policies and procedures including HIPAA standards.

QUALIFICATIONS

  • Current licensure as a Vocational Nurse in the State of California.
  • Experience working with psychiatric and dually diagnosed populations.
  • Proficient user of Microsoft Office Suite and EHR software.
  • Ability to problem solve and react appropriately to emergency situations.
  • Ability to work independently and effectively collaborate with a team.
