About the position:

Goodwin Home Health is currently seeking a full-time Home Health Lead Intake Coordinator. This position coordinates access to the Home Health program and is responsible for educating providers and the community about Home Health, as well as, lading the daily referral coordination of the intake team to ensure a streamlined admission process to Home Health services. The Home Health lead intake coordinator will support and assist the intake team with referral reviews via hospital portals, phone calls, emails, and faxes, and will aim to respond within 30 min of receiving the referrals. The lead intake coordinator will input/upload all clinical records and patient information into EMR and maintain a process to check entered items for accuracy. The lead intake coordinator will maintain a system for communicating with the intake and administrative team, all referring providers/agencies to support the mission of Home Health at Goodwin Living. Hospital discharge planning and Home Health knowledge is preferred. Will consider intake and admissions coordinator experience. Goodwin Home Health provides skilled care throughout Northern Virginia, focusing on the counties of: Arlington, Alexandria, Fairfax, Prince William and Loudoun.

Duties:

• Promotes the efficiency of the admission process by leading the intake team under the guidance and support of the Compliance Officer and Director of Clinical Services.
• Supports and performs the processing and evaluating referrals, determining eligibility in collaboration with intake coordination team, and Home Health management team as needed, entering the admission documentation in the chart, and assigning the admission/ start of care to the appropriate clinician.
• Supports the Compliance Officer with presiding over the Daily Standup with the billing team to determine eligibility, completion of chart for NOAs, and benefits for patients.
• Effectively communicates with case managers and clinician offices to obtain accurate notes, orders and documentation necessary to admit the patient.
• Obtain documentation to start and complete patient’s care to ensure Office Manager is able to create the Face-to-Face completion for compliance purposes.
• Participate in a teamwork approach, working with all disciplines and other roles involved in the patient’s care.
• Will be able to complete tasks in Microsoft Office Suite including Word and Excel to completeness and with efficiency.
• Always maintains confidentiality of patient and staff information.
• Attends mandatory meetings/in-services and maintains 100% compliance with mandatory data based in house training.
• Immediately reports abuse/suspected abuse to Compliance Officer, Director of Clinical Services or Administrator
• Performs additional job duties as assigned.

Qualifications:

• Two (2) years of home health, admission/ intake, case management or utilization review experience preferred.
• Proficient in Microsoft Office and Excel, ability to adapt to various EMR and hospital and referral portal systems currently used as well as other programs used throughout the community.
• Communication, documentation, and group dynamics skills
• Must be able to work with frequent interruptions while maintaining high quality accuracy.
• Willing to work beyond normal working hours and in other positions temporarily, when necessary.
• Always maintain privacy and confidentiality of patient’s information.
• Must be able to move intermittently throughout the workday.
• Must be able to cope with the mental and emotional stress of the position.
• Must possess sight/hearing or use prosthetics that will enable these senses to function adequately so that the requirements of this position can be fully met, to include use of departmental equipment and/or supplies.
• Must function independently, have flexibility, personal integrity, and the ability to work effectively with the residents/patients, personnel, and support agencies.
• Must be able to relate to and work with ill, disabled, elderly, emotionally upset, and at times, hostile people within the facility.
• May be required to lift, push, pull and move equipment, supplies, etc. up to 25 lbs. when necessary, per Office supplies, etc.
• Must be able to read, write, and speak English in a proficient and understandable manner.
• Must possess the ability to make independent decisions when circumstances warrant such action.
• Must possess the ability to deal tactfully with personnel, patients, family members, visitors, government agencies/personnel and the public.
• Must be able to follow oral and written instructions.
• Must have patience, tact, cheerful disposition, and enthusiasm, as well as be willing to handle residents based on whatever maturity level they are currently functioning.
