POSITION: Outreach Worker – Youth and Family Services

DEPARTMENT: ODAAT Youth and Family Services

REPORTS TO: Director of Youth & Family Services

POSITION DESCRIPTION:

One-Day-At-A-Time (ODAAT), a program partner of the Urban Affairs Coalition (UAC), is currently looking for an individual to fill the full-time position of Youth & Family Outreach Worker.

The Program Outreach Worker can expect to build and foster relationships with youth/young adults. Network and build community partnerships along with coordinators that deliver support and programming that consist of recovery planning, social development, and resiliency building. Through aspects of mentoring and case management services, the Outreach Worker will co-create service plans that will target growth in areas of home, school, and community. The goal for each is to enhance youth/young adult capacity to function in all aspects of living. Responsibilities for the outreach worker includes monitoring and providing updates on progress youth/young adults are making on their service plans, providing case management to individuals on a caseload, providing support at activities, crisis response issues, emotional support, and where appropriate services may be extended to children at home with their families.

The ODAAT Youth and Family Services Department works with at-risk youth and young adults to enable them to grow as individuals and become productive members of their community in- and out-of-school. This position is an exciting opportunity to work as part of a comprehensive youth engagement initiative which includes early intervention, prevention, and program development activities that are monitored through assessments and evaluations.

PRIMARY RESPONSIBILITIES:

  • Support Youth & Family Program Coordinators with delivering ODAAT services to at-risk youth/young adults and families.
  • Support Youth & Family Program Coordinators with supplying youth and families with general information and referral services for mental health, recreation, housing, employment, education, health and other services.
  • Work collaboratively with youth/young adults, parents, youth service providers, schools, city staff and law enforcement to develop effective outreach that ensures that youth have access to programs, services and opportunities needed for healthy development.
  • Initiate, develop and implement community outreach activities with youth/young adults and communities in Philadelphia. Facilitate connections between youth/young adults and their neighborhood and promote awareness of the positive role of youth in the community.
  • Document activity with youth and families and maintain reports that capture individual and group setting notes, ASAM criteria, pre and post data of evidence based material, biopsychosocial assessment, service plan and related met goals, and a discharge summary with noted outcomes and referrals.
  • Implement evidence-based practices and tools.
  • Able to meet a flexible work schedule including summer hours, and occasional evenings
    and weekends, and availability by telephone and/or cell phone during non-work hours.
  • Other agency related duties as required or assigned.

QUALIFICATIONS & SKILLS:

  • Bachelor's degree in a related field.
  • State and federal clearances required.
  • Minimum of 2 years of experience in youth mentorship programs, preferred.

LICENSES & CERTIFICATIONS

  • Valid Pennsylvania driver’s license preferred.

WORK ENVIRONMENT

The employee will work in an office and public, service-delivery, and school environment. Hectic situations can occur in the public area of the facilities characteristic to working with the youth and the related concerns and situations related to at-risk youth and young adults. Potential exists for volatile and unpredictable situations to occur. The requirements listed in this document are the minimum levels of knowledge, skills, or abilities.
