Since 1972, Therapeutic Health Services (THS) has provided evidence-based, culturally appropriate and compassionate behavioral health treatment, programs and services for children, youth, and families with substance use and mental health disorders. We are on the frontlines:

We are hiring a Wise Program Care Coordinator to join our Youth and Family branch in Seattle. As a WISe Care Coordinator, you'll play a crucial role in working with participants and families to assess strengths and needs, facilitating cultural discovery, and coordinating WISe Teams. With responsibilities ranging from monitoring care plan progress to providing linkage between multiple systems, your impact will be significant. The hours for this position is 9:00 am to 5:00 pm and the pay is $25-$27 an hour.

Requirements:
  • Works with participants and family members to assess strengths and needs across a number of life domains and participate in cultural discovery.
  • Collaborates with participants to identify significant others to be part of their WISe Team (the team ideally should be a combination of both professional and natural supports).
  • Monitors progress on the participant’s care plan and reviews progress with the WISe Team on a regular basis. Develops, in collaboration with the participant and family members, a crisis management plan that includes action steps and identifies specific responsibilities and communication procedures.
  • Coordinates WISe Teams and conducts CFT meetings with each participant and their team at least monthly, until no longer needed by the participant and family. Will also coordinate with the participant’s treatment team to incorporate participant and family “voice” in treatment activities and goals.
  • Provides linkage between the multiple systems within which participants are involved, e.g., child welfare, juvenile court, mental health, chemical dependency, special education and utilizes the full involvement and partnership of families and significant others in the care of the participant.

Minimum Qualifications:

  • Bachelor’s degree in a social service or related field required.
  • Completion of CANS certification within 30 days of hire. Completion of WISE training within 30 days of hire.
  • Minimum of two years’ experience in direct service work with “high-risk” youth, young adults, and families.
  • Reliable Transportation is a must
  • Obtain WA state certification as a Peer Counselor consistent with WAC 388-865-0107 or the ability to obtain certification within 6 months of hire.
  • Individuals who do not already hold a license or associate license through the Washington State Department of Health must submit Agency Affiliated Counselor application upon hire and be approved by the DOH.
  • Ability to complete online WISe training modules within 30 days of hire and be fully WISe trained within 6 months of hire.
