WellStone is North Alabama's most comprehensive behavioral healthcare provider serving over 13,000 persons annually throughout Northern Alabama, with offices in Huntsville and Cullman. We are a non-profit community organization that offers a wide range of services for every phase of life, from children to older adults.

Works with treatment staff to ensure outpatients are in good health while receiving outpatient care. Documents and communicates interactions and concerns to their treatment team and follows state regulations. This position is Full-Time Mon-Friday 8a-5pm in the Diversion/Adult Outpatient Program.

What we're looking for:

What you'll be doing:

  • Adheres to the Center’s philosophy of care, professional ethics, and other standards of policies and procedures
  • Utilize the nursing process in assessing, planning, evaluating, and providing care to residential clients
  • Administer stat IM or P.O. medications
  • Work under the Nurse Delegation Program regarding medications
  • Enter information into our Electronic Medical Records System
  • Interface with local laboratories to process ordering/requisitioning lab tests (per psychiatrist) or obtaining results from tests
  • Obtain and record vital signs, i.e., orthostatic blood pressure, weight, height, pulse rate, and rhythm
  • Perform crisis intervention as needed
  • In lieu of psychiatrist’s absence, conduct assessments of client’s mental status to include homicidal/suicidal ideation, appearance/grooming, affect, mood, energy level, rate/pitch of speech, perceptual disturbances, and physical status.
  • Medication monitoring to include assessing the effectiveness of given medications, changes in symptoms since the last visit, inquire about over the counter medications, observation of possible side effects, and document in the client electronic chart.
  • Educate client and family on medication, possible interactions, and side effects in addition to the importance of taking the medication
  • Complete client documentation per Center and State policies/procedures and standards in a timely manner
  • Evaluate and assess medical emergencies
  • Collaborate with Center staff regarding client well-being, and work with outside agencies as requested
  • Other nursing duties as assigned by the Division Director and Medical Director.
  • Full time, Mon-Fri 8-5

Miscellaneous & Physical Demands:

  • Maintain licensure in the State of Alabama
  • Complete and maintain documentation in a timely manner
  • Attend staff meetings
  • Maintain CPR/1st Aide, CPI Training, and other required training
