POSITION SUMMARY: Perform the duties required to promote healthy lifestyle changes in clients with chronic conditions, through education on the disease process, exercise, and nutrition. Provide instructions to clients in a place to facilitate the learning process in a more relaxed setting.
POSITION QUALIFICATION REQUIREMENTS

Education: High school graduate or equivalent. Medical experience preferred (Certified Nurse Assistant, Medical Assistant, Health Educator, etc.)

Experience: Minimum of one (1) year experience providing health education. Some general office experience required.

Skills and Abilities which may be representative but not all inclusive of those commonly associated with this position:
  1. Ability to implement a plan of care as devised by MD and RN.
  2. Strong decision making and prioritization skills.
  3. Ability to work with minimal supervision.
  4. Requires good written communication and organizational skills.
  5. Communicates effectively with health center team and clients.
  6. Exhibits excellent teamwork.
  7. Good computer skills including the use of word processing and e-mail software.

Transportation Requirement: Must possess a valid driver's license and automobile with insurance coverage as required by the State of Missouri.

License(s)/Certification(s) Required: CPR certification

FUNCTIONS OF THE JOB

Essential Functions
  1. Schedules and visits clients diagnosed with a chronic disease to assess their environment, needs, and available support. Completes accurate documentation and provides follow up information to the provider and nurse case manager regarding the visit.
  2. Provides clients with educational materials and answers questions.
  3. Schedules provider appointments, makes emergency visits to monitor patients and provide resources.
  4. Delivers resources, including food bags, education, and follow-up appointment reminder to clients' homes.
  5. Responsible for filing appropriate documentation in client files; monitors and maintains patient tracking system.
  6. Collaborate with health center staff to educate on chronic disease initiatives involving the health center teams.
  7. Collaborate with Patient Centered Health Home (PCHH) staff to provide resources, education, and follow-up to PCHH patients, Nurses, and Health center providers.
  8. Assist with quality measures, and health and wellness initiatives.
  9. Recruit for, schedule, and facilitate patient health education groups.
  10. Facilitate, assist, and follow-up with community referrals.
  11. Assist with implementation of programs/initiatives.
  12. Assist with facilitation of referrals for high-risk patients.
  13. Other duties as assigned.
Marginal Functions
  1. Participate in community health fairs and events.
  2. Maintains accurate travel log.
