Job Description
We are seeking a dedicated and compassionate Home Health / Hospice Aide to join our team for the 1st shift, Monday to Friday. As a Home Health / Hospice Aide, you will play a vital role in providing essential care and support to our patients in the comfort of their homes. Provide personal care and light housekeeping services to terminally ill patients in the home, nursing facility, inpatient units or other environments where the hospice or home health patient resides, in accordance with an established plan of care and under the supervision of a registered nurse. Actively participates as a member of the hospice interdisciplinary team. Provides care and comfort to patients in accordance with assigned tasks and an established interdisciplinary and hospice aide plan of care. Observes the patient, reports and documents observations and care performed. Provides timely documentation of services based on the patient’s care plan.

Primary Duties:

  1. Ensure the quality and safe delivery of personal hospice services to the hospice patients while also supporting their families and caregivers.
  2. Perform or assist patient with personal care activities as prescribed in the care plan, including but not limited to hygiene, mobility and elimination. If unable to perform a certain task, reports to the Home Health/Hospice Aide Supervisor.
  3. Provide and maintain safe environment for patient, by using equipment safely and properly such as footstools, side rails, oxygen, etc.
  4. Practices universal precautions in all contacts with patients.
  5. Perform or assist patient/family with home maintenance activities as prescribed in the care plan including, but not limited to light housekeeping and meal preparation.
  6. Perform support activities as prescribed in the care plan including but not limited to supporting independence, providing companionship, and providing respite for families.
  7. Respect the autonomy and right to confidentiality of all patients and families.
  8. Monitor adequate fluid and nutrition intake, temperature, pulse, respiration, skin condition and basic elements and changes in body functioning and record/reports appropriately.
  9. Practice safe transfer, ambulation, normal range of motion and positioning techniques.
  10. Identify need for intervention by other members of the interdisciplinary team on an ongoing basis and reports to RN case managers.
  11. Document care provided and response of patient/family to care and submit documentation in a timely manner in accordance with agency policies.
  12. Assist patient/family/caregiver and other team members in providing continuity of care.
  13. Work in cooperation with family/caregiver/and interdisciplinary team members to meet the emotional needs of the patient/family/caregiver.

Qualifications:

Education:

  • High school diploma or equivalent.
  • Certified Nursing Assistant (CNA) or Home Health Aide (HHA) certification is required.
  • Must demonstrate skills and have a passing score on a written competency test in accordance with 42 C.F.R. Section 484.36 prior to working independently with a patient.

Experience:

  • Minimum age requirement of 18.
  • Minimum of 1 year of direct patient care experience in an institutional or home care setting required with community health/hospice experience preferred.
  • Ability to work within an interdisciplinary setting.
  • Understanding of Hospice philosophy and needs of terminally ill.

Licensure, Registration, and/or Certification:

  • Valid driver’s license and reliable transportation that is insured in accordance with Pennsylvania requirements.
  • Must pass background checks and drug screening.
