This is a full time, 40 hours per week, position in our Shenandoah Day Hab. The hours are Monday- Friday 8am-4:30p.

Day Hab Instructor


I. The Day Hab Instructor is responsible to the Director of Day Services and/or Director of Day Services.


II. This is a full-time, paraprofessional position.


III. Specific Job Duties & Responsibilities:

1. Provide direct individual training to develop/maintain skills in areas identified and requested by the individual’s team. Provide training in community settings.

2. Responsible for providing a supportive and therapeutic atmosphere for the individuals receiving Day Habilitation Services.

3. Assist persons served in the development and implementation of personal goals and objectives, and methods for achieving them. Carry out instructional activities consistent with the individual’s training objectives.

4. Document individual performance including activities pursued and behaviors observed. Keep all charting, incident reports and daily reports up to date and accurate.

5. Evaluate individual performance on an on-going basis.

6. Complete IPP and Progress information regarding the individual’s performance/participation in the program, and return to the
Program Coordinators.

7. Discuss with the Director of Day Services any problems/needs, significant changes, and accomplishments the individual has displayed.

8. Counsel individuals whenever the need arises and/or refer them to their Program Coordinators when the need indicates.

9. Substitute for other Day Hab Instructors as needed.

10. Drive van routes as assigned.

11. Assist with recreational activities as needed.

12. Provide appropriate training and supervision when individuals are in community settings.

13. Obtain prior approval for all purchases of needed supplies.

14. Provide first aid and medical attention where needed and/or alert Program Coordinator and Director of Day Services of the situation as needed.

15. Responsible for scheduling activities/outings, and reporting any concerns or activities suggested to the Director of Day Services.

16. Participate in regularly scheduled staff meetings.

17. Participate in agency's overall safety program. This includes becoming familiar with emergency drill procedures, training in CPR, and first aid procedures.

18. Responsible for being familiar with established behavior management techniques and crisis intervention strategies to insure the physical safety of the individuals in the immediate area and self during a behavioral crisis situation.

19. Responsible for displaying a willingness to participate in working with and training of individuals with behavioral problems.

20. Participate and cooperate with agency investigations.

21. Handle any other duties as assigned by the Director of Day Services and/or Executive Director.

IV. Essential Functions:

1. Provide training and supervision in the development of identified skills.

2. Provide a supportive and therapeutic atmosphere.

3. Complete all data collection on goals for individuals served.

4. Complete all progress and Individual Training Plan documentation (i.e., data collected on goals for persons served) to be turned
into Program Coordinators.

5. Assist persons served in the development and implementation of goals and objectives.

6. Drive designated van routes as assigned/requested by Director of Day Services.

7. Perform break duties as assigned.

8. Maintain compliance with Department of Transportation and agency Drug and Alcohol testing policies and procedures.

9. Obtain medication certification as required.

10. Nishna Productions, Inc. exists to serve the needs of persons with disabilities. Frequently,
these persons have behavior disorders which may result in the display of aggressive outbursts of a physical nature. The person served may or may not be able to control these episodes. Each employee is required to physically assist in dealing with these behaviors if the situation warrants.

11. Train persons served in the areas identified and requested by the individual’s team.

12. Administer tests that are deemed pertinent to the assessment of client's skill.

13. Ability to complete electronic documentation.

14. All staff/volunteers of Nishna Productions, Inc. will comply with state and federal regulations regarding HIPAA (Health
Insurance Portability and Accountability Act).

15. Every employee/volunteer of Nishna Productions, Inc. is on a need to know basis regarding the Protected Health Information (PHI) of the individuals they serve. Therefore a security clearance level is assigned for each job description and those security levels are allowed access only to the information required to complete the duties in each position.

· Level One allows the least access and is intended for truck drivers, maintenance staff, redemption center workers, and volunteers.

· Level Two is for direct care workers including; floaters/substitutes, SCLS, Day Hab Instructors, Day-Hab aids, Vocational Trainers, lead staff, and 1:1 staff.

· Level Three includes the office support staff.

· Level Four is for Team Leaders, supervisors, and Work Center Supervisors.

· Level Five is the security clearance given to Program Coordinators.

· Level Six allows access to all PHI and includes Directors/Administrators, Quality Assurance, and IT staff.

· Temporary clearance for PHI access can be given if staff require the access to complete extra assigned duties (i.e. filling in for another department). Maintenance personnel are given permanent access to areas where PHI is stored but they may
need to enter in order to complete their duties.

V. Qualifications/Requirements

1. Prefer previous similar job experience in teaching or instructional-type setting, or an A.A. or B.A. Degree.

2. Prefer one-year experience as a direct care staff member in a human services agency, or in an educational agency.

3. Must be able to relate to and understand individuals who have physical and mental challenges.

4. Must have ability to meet and deal with the general public.

5. Personal adjustment and behavior must be of such quality that they provide an appropriate adult model for the individuals.

6. Exhibit above average writing and verbal communication skills.

7. Willingness to participate in behavior management techniques and crisis intervention strategies to insure the physical safety of
the individuals and staff in the immediate area and self during a behavioral crisis situation.

8. Ability to communicate with co-workers in an effective and courteous manner.

9. Uphold all confidential information concerning individuals or the organization.

10. Ability to be trained in how to administer any assessment tools deemed necessary by the agency.

11. Ability and desire to work with other employees as a team.

12. Must be flexible and willing to work scheduled hours as required to do the job.

13. Must not have been legally convicted of any type of assault, abuse, or bodily injury.

14. Must conform to established DOT and agency Drug and Alcohol testing policies.

15. All agency staff are required to participate and cooperate with agency investigations. Confidentiality will be respected.

16. Maintain valid Iowa Chauffeur's License and have a positive driving record. Must be considered insurable by agency's
insurance carrier.

17. Individuals "must not have or be a carrier" of a serious infectious or communicable disease, such as, but not limited to,
incurable, fatal, or debilitating diseases, which cannot be eliminated or reduced by reasonable accommodation. A current example would include, but not be limited to, tuberculosis. Determination of the existence of a serious disease is to be made by the proper medical authorities.

VI. Physical Requirements:

1. Must be able to travel from place to place, up to 70 miles.

2. Must have stamina and physical ability to load and unload, self and clients from vehicles.

3. Must be able to be physically active (walking, etc.) for periods of time not less than 10 minutes in a period.

4. Must be able to bend, squat, push, and pull.

5. Must be able to lift 50 lbs.

6. Must be able to provide physical care necessary to individuals when performing direct care duties.

7. Ability to physically assist an individual to regain his/her balance, or to transfer from a sitting, standing, or prone position.

8. Demonstrate ability to perform routine cleaning tasks, such as vacuuming, mopping, and the use of cleaning agents, meal preparation, and clothing care.
