SAFY is seeking a full time Family Specialist for our Fort Collins, Colorado Foster Care Team.
As a Family Specialist, you will be responsible for providing a range of moderately complex clinical services with children, adolescents, and identified adults. May also assist families in coordination and identification of resources and assist with case management.

What You’ll Do

Clinical:

  • Provide a range of moderately complex clinical services with children, adolescents, and adults. This will include diagnostic assessment, treatment planning, and psychoeducation, tangible skill building, support, and crisis navigation.
  • Administers, scores, and interprets moderate level psychometric assessment instruments.
  • Participates in the development of the Individual Service Plan and provides ongoing mental health services.
  • Provide short-term Intensive Services to youth in crisis. Will follow intensive, in-home evidence-based service model based on Family Wellness and other Trauma Informed approaches. Services are provided at the level of frequency, intensity, and duration appropriate to prevent placement or achieve reunification. Services provided are tailored to each family’s needs, problems, strengths, lifestyle, and culture.
  • Develop/Maintain consultative and liaison relationships with catchments area caregivers (e.g., schools, human services agencies, etc.) to cultivate work relations, provide consultative services and/or promote the coordination and development of mental health services. Establish mutually respectful rapport and implement interventions necessary to meet needs of client/collaterals; appropriately set limits and maintain helping role as a Mental Health practitioner.
  • Complete assessments as required by state rules, agency policies, and/or contract requirements. Assessments may include psychosocial assessments, Diagnostic Assessment Forms, and independent living skills assessments.
  • Prepare Individualized Treatment Plan (IAP, ISP etc.), DAF’s, and Mental Health Individualized Services Plans according to established time frames involving all appropriate parties as required by policy and regulations of the state.
Compliance/Reporting:
  • Timely document outcome of home visits, support sessions, interaction with parties involved in the client’s care and incidents as required by policy, regulations, and contract requirements. Prepare and submit timely all required company, RFP, program, referral agency and regulatory reports, progress notes, and billing tickets. Compile profile using pre and post assessment tools for outcome measures. Report census information to DOM/finance department for billing as indicated.
  • Maintain contact with referral agency caseworker using discharge reports, Diagnostic Assessment forms, telephone calls, and incident reports in addition to regularly scheduled and required written reports.
  • Review all youth and home files pertaining to caseload as part of the quality improvement process.
  • Follow monthly and quarterly requirements as defined annually by STARS standards. Meet utilization standards for Mental Health services as defined in the Utilization Policy.
  • Inform clients of company policies, procedures, rights, and responsibilities. Enforce policies by writing corrective action plans, incident reports, and conducting rules investigations.
Other:
  • Establish and maintain a positive working relationship with all levels of SAFY staff by participating in joint meetings, and problem identification and resolution.
  • Establish and maintain a supportive and positive working relationship with youth, foster parents, referring agencies, and other SAFY stake holders.
  • Attend relevant in-house and external seminars, trainings, conferences and workshops to develop and enhance knowledge in the designated area as well as achieve relevant education hours per year to enhance professional and job-related skills.
  • Attends required meetings within division, region, and central office (team and/or community partners).
  • Participate in assigned committees and chairs if so appointed. If appointed, maintains liaisons with state and national organizations.
  • Meets regularly with supervisor to discuss issues and performance.
  • Submit monthly/quarterly reports to immediate supervisor.

Qualifications
  • Master’s degree in the field of social work, counseling, marriage and family therapy or other approved related field of human services. Enrollment in such program with expected graduation in one year or less (CO only)
  • Bi-Lingual Highly Preferred
  • Two years of relevant experience preferred in the field of mental health/therapeutic intervention with children, adolescents, and families. Experience with family preservation services and best practice In-Home models is preferred

  • Targeted hiring range: $50,000-$62,000
    • Compensation is commensurate with transferrable experience, education and licensure of candidate, location of the position, along with internal equity and budgeted amount for the role.
