Position Details

Family First is growing because people learn about our unique set of family-centric services. The Home Care Coordinator provides mentorship, training, one-on-one guidance, and support to in-home CarePros while providing excellent customer service and professional care to clients and offers administrative assistance. Consistently matches field employees with clients based on the needs and field staff’s skill level and attributes.

Job Duties

CarePro Support:

  • Provide supervision of direct care staff (CarePros) including hiring, training through skills evaluation and orientation, mentoring and coaching/counseling
  • Accompany the first scheduled CarePro with each new client- virtually or in person
  • Meet and become familiar with all current clients and CarePros that they supervise
  • Conduct supervisory visits for CarePros and clients on a routine and ongoing basis
  • Provide training to CarePros under the direction of the Manager of Home Care or Professional Care Manager
  • Monitor and update appropriate business trackers on a regular basis
  • Tracking and compliance of CarePro continued education and training programs
  • Responsible for follow up with CarePros and clients
  • Coaching CarePros as needed
  • Schedule CarePro annual reviews with approval of the Manager of Home Care or designee

Client Support:

  • Conduct new client assessment in accordance with new client onboarding process
  • Develop customized care plans for each client and ensure the assessment is updated regularly to reflect the plan
  • Obtain input from client, client’s family, and other professionals involved with client’s care to maintain optimal plan of care
  • Function as the primary point of client/CarePro contact, addressing issues/concerns using judgment consistent with standards, practices, policies, procedures, regulation, or law.
  • Appropriately document client and/or CarePro communication with timely updates to client care plans in all applicable systems and/or processes
  • Complete delegation with all current clients requiring a delegated CarePro
  • Function as point of contact for all current and future clients and CarePros
  • Participate in regularly scheduled on-call rotation to handle off-hours staffing needs
  • Be available as backup CarePro for call-ins and last-minute shifts

Staffing Support:

  • Efficiently schedule CarePro/client based on CarePro skill set and client needs
  • Utilize effective staffing skills to strategically staff CarePros avoiding overtime whenever possible
  • Regularly monitors the Home Care department’s monthly staffing statistics and develop solutions to increase client hours and satisfaction
  • Maintain communications with client, client’s families, or guardian regarding changes in scheduling hour or staff.
  • Ensure proper CarePro rates are assigned, and appropriate billing codes are used
  • Ensure Clear Care system information is current and up to date Ensure all approved CarePro time off requests are updated in Clear Care and reported to the payroll department for PTO hours to be paid
  • Billing and payroll for client and Carepro caseloads

Other Responsibilities:

  • Ensure compliance with WAC & RCW for self and assigned CarePros
  • Maintain flexibility and reliability; travel within assigned service areas as needed and expected; provide overall support to the home care department as needed
  • Provide back up to recruiter, reception desk, intake and CarePros as needed or as assigned
  • Assist with performing new client assessments and intakes
  • Assist with the CarePro recruitment process; screening, interviewing, and evaluating for hire
  • Complete all Continuing Education courses as required by WA State for Family First as a home care agency
  • Assist with the recruiting process and new hire orientation as needed
  • Assistance to office assistant for front office/reception
  • Must be able available for emergency client coverage and be able to follow all care plan tasks assigned
  • Participates in case reviews, consulting with the team caring for the client, to evaluate the client’s needs and plan for continuing services.
  • Adhere to all guidelines and policies outlined in Family First Employee Handbook
  • Travel within the greater Spokane/Tacoma/Seattle area as needed or assigned
  • Preforms any other duties as needed or assigned

Qualifications

  • Minimum two years supervisory experience required
  • Current Certified Nursing Assistant or Home Care Aide Certification required
  • Nurse Delegation experience preferred
  • Diabetes Delegation preferred
  • Current CPR certificate or willingness to obtain certification within 14 days of employment
  • Ability to travel required within the assigned service area
  • Comply and maintain valid Driver’s License, vehicle insurance and clean DMV record
  • Strong computer and technology acumen
  • Strong leadership skills with an elevated level of professionalism

Compensation: $24-27/hour depending on relevant experience.

Hybrid – Regular travel to client’s homes is required

Schedule:

  • FT- 8-hour shift 8:00am - 4:30pm
  • Work days - Monday- Friday
