Job Summary:

Responsible for providing individual screening, counseling, intervention and follow up through education, therapy, short-term problem resolution, advocacy and referral for at-risk elders admitted to the Older Americans Act Counseling Program.

Duties and Responsibilities:

  • Respond to individual referrals to the Older Americans Act Counseling Program.
  • With informed consent, conduct client pre-screenings (or screenings if a referral) and complete prioritization form to determine eligibility for the program.
  • Provide specialized individual counseling using techniques appropriate to an ongoing caseload with a caseload size appropriate for scheduled hours.
  • Facilitate a biopsychosocial assessment within 30 days of intake; develop a treatment plan within 45 days of intake and complete treatment plan reviews every 6 months thereafter; establish goals, specific objectives, interventions, outcomes, and time frames for treatment of each assigned client.
  • Integrate mental health interventions with the overall service and supports to enhance emotional and behavioral functions.
  • Attempt to generate change and modification in client’s behavior and attitude to facilitate personal growth in accordance with best practices.
  • Refer to other community support services for ancillary client needs and for the client’s ongoing treatment.
  • Utilize approved Telehealth platforms when possible or travel to locations throughout Pinellas and Pasco counties.
  • Review Treatment Plans, Assessments, Billing documents, Caseload schedule and the files to ensure compliance.
  • Complete documentation in a concise and timely manner (within 2 days of activity to be documented), including but not limited to, administration, clinical or caseload requirements.
  • Provide quality services and maintain productivity level as defined by the supervisor.
  • Conduct community outreach and establish collaborations with agencies in the aging network.
  • Any other duties as assigned.

Education:

Master’s degree in psychology, psychiatry, social work, marriage and family therapy, mental health counseling, or any licensable degree under the Florida Statues 490 or 491. Must possess Clinical social work, marriage and family therapy, or mental health counselor license or registered intern license by the Department of Health in accordance with Chapter 491, Florida Statutes.

Skills/Qualifications:

Experience with seniors, gerontological issues and some type of supportive counseling is required. Knowledge of geriatric human development and individual counseling skills; knowledge of substance abuse issues and intervention techniques and practices; knowledge of social and emotional problems associated with aging; knowledge of community organizations and resources; ability to deal with clients who may be verbally aggressive are highly preferred. Computer skills (Excel and Word) are required.

Special Requirements: Must pass Level II criminal background screening. must sign Medicaid Attestation Payroll Form per Department of Elder Affairs/AAAPP requirements. Must sign SMMCLTC Program – Prohibited Activities.
