We're hiring a Home Health and Hospice Certified Nursing Assistant / CNA to join our growing team, providing care to patients residing in King County.

JOB SUMMARY:

Responsible for providing limited nursing care and therapy services to Home Health and Hospice patients under the clinical supervision of a Registered Nurse (RN) and may receive guidance from a Physical Therapist (PT), Occupational Therapist (OT), or Speech Therapist (ST) as outlined in the Aide Care Plan;
Performs basic technical skills which may include CPR; taking and recording vital signs; change non-sterile dressings; measuring and recording fluid intake/output; assist patient with taking oral medications as directed by the physician or nurse;
Assists patient with personal care which may include bathing; mouth and skin care; grooming and dressing; assistance with bed pans and/or urinals; eating and hydration;
May assist with the use of special equipment (i.e. walker, wheelchair, cane);
Takes prompt action in response to changes in patient’s health condition and notifies RN/therapy case manager and/or Clinical Manager in a timely fashion;
Effectively communicates with patients, caregivers, RN/therapy case manager, Clinical Manager and other members of the interdisciplinary team to assure an appropriate, quality and smooth flow of services for the patient through the care continuum;
For incorporation into the Home Health/Hospice record, provides accurate, legible and timely clinical documentation in compliance with regulatory, quality and Agency guidelines;
Provides patient care in an uncontrolled home environment;
Other duties as assigned.

EDUCATION AND EXPERIENCE REQUIREMENTS:

Current CNA license in the state of operation is required
Experience as a Home Health Aide and/or Hospice Aide preferred
Ability to care for patients in a wide variety of environments
Able to lift objects weighing over 80lbs
Current BCLS certification
Current health certificate/physical examination and TB testing
Valid Driver’s license and reliable automobile
Food Handler’s card required at date of hire
Proof of current automobile insurance (MVR checked)
Criminal background check and national sex offender clearance
Must have proof of up to date COVID vaccine, or submit an exemption request to be reviewed/approved prior to hire
