A geriatric care manager is a healthcare professional who provides care and social services for the elderly. Geriatric care managers may work in hospitals, medical facilities, or elderly care homes. They may also visit clients at their residence. Geriatric care managers help their clients and families to evaluate any special needs and conditions, design and implement long-term care plans, and choose medical professionals to aid in the plan. They develop close relationships with their clients and help them deal with physical, emotional, and mental issues.
