Therapist -LCPC/LCSW-C

Wisdom HealthCare Services is seeking to add energetic, team-oriented Therapist (LCPC)/ LCSW-C

Responsibilities:

• Establish open lines of communication for individuals with mental or emotional issues
• Offer assistance to individuals with substance abuse issues
• Provide prevention-oriented therapy
• Implement and review treatment plans
• Collaborate with additional treatment providers and placement agencies
• Ensure all documentation is completed in a timely manner

Qualifications:

• Previous experience in mental health counseling or other related fields
• Familiarity with community mental health resources
• Excellent written and verbal communications skills
• Ability to build rapport with clients
• Strong leadership qualities

Summary

• Provide individual and/or family counseling sessions to address substance use disorders, adjusting to life, and making changes. Coordinate care with therapists and other team members.
• Complete and maintain accurate records and reports regarding the patients' histories and progress notes and services provided, and other required information.
• Collaborate with the patient to build individualized treatment plans that identify specific and measurable goals for treatment.
• Review and evaluate clients' progress in relation to goals described in treatment plans.

Responsibilities

• Intake and referrals to receive authorization
• Ensure that all assigned patients show up and participate in all required sessions
• Identify substance abuse related issues, create corresponding goals and treatment plans, and maintain treatment plans per policy
• Introduce patients to evidence-based interventions and coping mechanisms
• Assist patients in reestablishing their standing in the community
• Provide group IOP counseling sessions
• Provide updates and progress reports to courts as required
• Refer patients to support groups as required and indicated in the treatment plan
• Participate in the creation of aftercare and transition plans
• Accurately maintain all required documentation within the electronic health record
• Complete and maintain all required insurance documentation
• Participate in required team meetings, including treatment team, staff meetings, and other ad hoc meetings as assigned by supervisor
• Provide case presentations to the treatment team when required
• Participate in supervision on a weekly basis with an assigned supervisor as required per licensing board
• Ensure all documentation is completed in a timely manner

Qualifications

• 1+ years of previous experience in IOP or PRP, mental health counseling, or other related fields
• Familiarity with community mental health resources
• Excellent written and verbal communications skills
• Ability to build rapport with clients
• Strong leadership qualities
