divh2HHA - Iris/h2pIris Home Health Care Agency - West Palm Beach, FL 33407/ph3Job Responsibilities/h3pDuties of the Home Health Aide (HHA) include, but are not limited to:/pulliProvides assistance with personal care, hygiene and activities of daily living./liliEncourages client participation in activities to the extent to which the client is able/liliPerforms duties of a Home Health Aide (HHA) as per: Scope of Practice and Florida Law/liliAssists client to: a.)bed, b.)commode, c.)chair, and assist with ambulation/liliTurns and positions bed bound clients/liliMeasures and records intake/output, as assigned/liliMeasures and records temperature, pulse and respiration on each visit/liliChanges bed linen if needed/liliPrepares simple meals following dietary instructions as instructed/liliMaintains a neat and clean environment/liliMay grocery shop one time a week for a list of ten items or less as needed/liliInforms supervisor of any changes in clients condition or home situation/liliFollows care plan as written/liliProvides documentation of care given on Agency approved forms/liliReports any changes in patients condition, living conditions etc., to DON/Supervisor as they occur/liliPerforms any other task/duty that is specifically assigned by supervisor, and for which Home Health Aide (HHA) has been specifically trained. Documentation of specific training must be included in employees personnel file/liliConducts self in professional manner at all times and in all situations/liliProvides Agency with: required certifications, required license, necessary information to be able to verify experience, documentation of CEUs/liliAccepts only those assignments for which he/she is qualified as per Florida Law/liliComplies with all Agency policies and procedures/liliCommunicates with Agency about any problems or concerns/liliComplies with HIPPA Privacy Rules, Policies and Procedures/liliReports any suspected violations of Privacy Practice to Privacy Official as soon as breach/possible breach is identified./li/ulh3Qualifications/h3pJob Qualifications/pulliMust provide evidence of formal training/liliMust provide physical Home Health Aide (HHA) certificate/liliMust provide documentation of clearance to work (green card, employment authorization card, social security, etc...)/liliMust provide evidence of competency training and evaluation as well as evidence of at least annual attendance at in-service education programs/liliMust be able to use Electronic Visit Verification (EVV) program on own cellular/ mobile device/liliMust be able to provide own transportation/liliMinimum of one (1) year current experience is required/li/ul/div
