Nuvance Health, formerly known as Health Quest Medical Practice is seeking a Nurse Practitioner to work in collaboration with the Medical Oncology team at Vassar Hospital in Poughkeepsie, NY. This position will coordinate, oversee and provide advanced nursing care to medical oncology patients and functions as a Clinical Patient Navigator for this population.

Job Highlights

+ Works collaboratively with physicians and nurses and is responsible for providing comprehensive care to patients.

+ Conducts comprehensive and episodic heath history and physical assessment of patients.

+ Oversees treatment infusions and supportive care needs.

+ Writes orders within the limits of the practice agreement.

+ Maintains clinical documentation according to group practice.

+ Participates in clinical quality and research as needed.

+ Educates patients, families and caregivers on treatment plans.

+ Participates in learning opportunities.
