SUMMARY DESCRIPTION Provide clinical care and services to patients in specialty areas as assigned as qualified to perform. Function as a fully accountable colleague with physicians and clinics through collaborative care agreements DUTIES

The following duties are typical for the APRN. Incumbents may not perform all of the listed duties and/or may be required to perform additional or different duties from those set forth below to address business needs and changing business practices.

PRIMARY ACCOUNTABILTIES

Achieve Results

  1. Effect improvements in patient well-being through accurate and timely diagnosis and delivery of relevant, high quality medical care. Ensure the delivery of competent, accurate, medical care and treatment to all patients as assigned.
  2. Meet or exceed all measurable standards of care, including overall clinical, quality, and productivity goals.
  3. Ensure accuracy, efficiency, and appropriate confidentiality with patient charting and related recordkeeping and administrative functions as assigned.

Operational Excellence

  1. Ensure the delivery of all medical treatment and care is consistent with all regulatory, accreditation, and professional standards, including clinical policies and quality initiatives.
  2. Understand and ensure all practices and procedures related to the delivery of medical services are consistent with contracted protocols and procedures.

Relationship Management

  1. Develop and ensure favorable relationships with all patients and their families. Achieve commitments from patients to adhere to an effective plan of care, and ensure all patient services are delivered in a caring and professional manner.
  2. Develop and ensure ongoing, positive relationships and collaboration with other physicians, nurses, administrative and clinic operations staff. Ensure the success of each clinic operation through collaborative support and working relationships with all clinic operations staff.

Professionalism & Stewardship

  1. Ensure all actions, job performance, personal conduct and communications represent the organization in a highly professional manner at all times.
  2. Uphold and ensure compliance and attention to all corporate policies and procedures as well as the overall mission and values of the organization.

PRIMARY TASKS & DUTIES

  1. Comprehensive physical assessment; evaluating, diagnosing, and treating new and existing patient's medical conditions.
  2. Initiating and interpreting labs and x-ray studies including CTs & MRIs.
  3. Performing medical procedures.
  4. Prescribing and referring patients for specialized consultation.
  5. Assisting Physicians in medical procedures.
  6. Using prescriptive authority to write prescription medicines for patients.
  7. Write/Dictate progress notes on patients' charts indicating patient status and treatment procedures performed.
  8. Conducting follow-up patient care.
  9. Providing health education to patients and families.
  10. Supervising and/or coordinating the activities of patient care and support staff within the health care center.
  11. Teaching and training illness prevention.
  12. Actively participate in community health education.
  13. Performing emergency life saving procedures in cases such as cardiac arrest, respiratory arrest, massive hemorrhage, or similar emergencies.
  14. Are among front line medical providers in emergency disaster services.

EDUCATION, EXPERIENCE & QUALIFICATION GUIDELINES

Any combination of education and experience that would likely provide the required knowledge and abilities is qualifying.

  • Advanced degree and relevant certification in medicine.
  • Valid (State) medical license (RN, LVN, FNP, etc.) required.

Performance Requirements Ø Courteous, honest and professional at all times.Ø Able to communicate and relate well with physicians, clinical support staff and other HKH employees.Ø Able to provide innovative input into the development of the clinic and its processes.Ø Maintains appropriate licensure and certification including meeting and/or exceeding CME requirements.Ø Efficient, organized and accurate.

PHYSICAL DEMANDS AND WORKING ENVIRONMENT

Working Conditions: Medical office environment. Multi-task oriented position including direct patient care may require sitting or standing for long periods. Some bending, stooping, and lifting (up to 50lbs). Must interact with physicians, patients and other office personnel. Must possess socio-economic and cultural sensitivity. May involve contact with angry, upset or ill persons. Will involve potential exposure to blood and body fluids and other hazardous substances. Must have visual acuity and manual dexterity to assist with procedures, perform and interpret lab results, and interface with computer. Must have auditory acuity to handle phone calls and extensive patient interaction.
