Core Health Partners, (CHP) is a community-based organization that delivers services within the spectrum of prevention, restoration, and rehabilitation. The CHP model is built upon a seamless integration of (a) clinical care, (b) medical-based fitness, (c) community-based resources, and (d) self-management coaching and education. CHP is supported by a team of physical therapists, nurses, dietitians, health coaches, and fitness experts delivering physical and aquatic therapy services, chronic disease management programs, and health coaching. CHP works collaboratively with physicians, employers, universities, and a variety of community partners. CHP is currently seeking a full-time Physical Therapist who will be a part of our focus to provide an integrated clinic to community model. The Physical Therapist will evaluate the needs of referred patients, formulate treatment plans, and provide and oversee the physical/aquatic therapy services as defined in treatment plans. This position provides direction and instruction to individuals who have specific needs due to chronic disease conditions and/or physical disabilities/injuries and will oversee a staff of PTAs, aides, and student interns .

GENERAL FUNCTION: This position has an emphasis on working with general population with musculoskeletal, and chronic diseases that include diabetes, obesity, Parkinson’s, arthritis, Alzheimer’s, and vascular disease.

RESPONSIBILITIES INCLUDE: - Perform physical and aquatic therapy patient care services adhering to APTA guidelines for physical therapy best practices & code of ethics.

- Comply with Florida Board of Professional Licensure with respect to licensure requirements and supervisory rules and regulations.

- Examine patients and develop a plan of care that promotes movement, reduces pain, restores function, and prevents disability.

- Work with the patient, family members, and other health care providers to ensure the goals of the plan of care are met and the patient outcomes are optimal.

- Promote and engage patients in healthy lifestyles by developing fitness and wellness-oriented programs for healthier and more active lifestyles.

- Demonstrate appropriate and effective written, verbal, and non-verbal communication with patients and staff to protect private health information.

- Complete all documentation in a timely manner.

- Communicate timely with referring physicians regarding patient treatment plan, progress notes and physical therapy discharge.

- Appropriately operates equipment related to delivery of physical therapy procedures and treatments.

- Follow equipment maintenance protocols and communicate potential problems with equipment to supervisor in a timely manner.

- Store and secure equipment and supplies as required for patient treatment sessions.

- Serve as a staff point of contact for general assistance and emergency situations.

THE HISTORY OF CORE HEALTH PARTNERS, CHP
Core Health Partners started as Florida Based Affiliate of Community Health Partners, CHP.

CHP originally formed in Des Moines, Iowa by a group of YMCA professionals who are credited with designing, building, and operating the nation’s first medically integrated YMCA (Clive, Iowa). This YMCA facility operated with their own National Provider Identifier (NPI) and first used the term “YMCA Healthy Living Center”, a title is now commonly used among YMCAs.

Community Health Partners led the clinic to community movement in 2017 when they spread their clinical practices into a handful of YMCAs in Iowa and in a little less than a year the decision was made to open the Core Health
Partners in Florida. The Florida operation was led by a former YMCA CEO.
