**Job Title: Home Health Aide**

**Job Overview:**
As a Home Health Aide at A Place at Home West Chester, you are a crucial part of our care team, dedicated to improving the quality of life for our clients. Your role will involve providing compassionate, high-quality care, and assistance with activities of daily living (ADLs) in accordance with each client's individualized care plan. Under the supervision of our registered nurses, you will ensure our clients receive the best care possible in the comfort of their own homes.
**Key Responsibilities:**
- Providing assistance with ADLs, such as bathing, dressing, grooming, and toileting.
- Assisting with meal planning and preparation, observing dietary restrictions when necessary.
- Assisting with medication reminders in compliance with established care plans.
- Performing light housekeeping tasks such as dusting, vacuuming, and laundry to maintain a clean and comfortable living environment for our clients.
- Assisting with mobility and transfers, ensuring client safety at all times.
- Providing companionship and fostering a nurturing relationship with our clients.
- Documenting and reporting changes in client's physical condition, behavior, or living conditions to the supervising nurse.
- Following HIPAA guidelines and maintaining the privacy of all client PHI (Personal Health Information).
- Participating in ongoing training and professional development opportunities.
**Skills/Qualifications:**
- Completion of state-approved Home Health Aide certification course.
- Strong communication skills, both written and verbal.
- Compassionate and empathetic with a strong desire to improve the lives of others.
- Ability to work independently and maintain high levels of patience and understanding.
- Physically able to perform the duties of the position, including lifting and physical movement as necessary.
- Reliable transportation and valid driver's license.
**Education/Experience Requirements:**
- High school diploma or equivalent.
- Must have valid Home Health Aide certification.
- Must pass a background check and drug test.
**Physical Demands:**
The position involves moderate physical activity such as bending, lifting, and walking.
