We are seeking a dedicated and licensed Clinical Social Worker to join the Intertwined Trauma-informed Therapy Clinic as a full-time remote practitioner. This role is key in delivering exceptional mental health care to individuals facing challenges due to trauma and adversity. You will be instrumental in assessing, planning, and executing treatment strategies that foster healing and resilience, with a focus on all aspects of mental health in a supportive and understanding manner.

This position is fully remote and allows for flexible scheduling, making it possible for you to design your work hours to fit your personal life while ensuring coverage for our patients. While operating from a hospice center, your role will demand both empathy and professionalism as you engage with clients remotely who may be navigating complex emotional and psychological landscapes.

• Conduct thorough mental health assessments, including psychosocial evaluations and diagnostic interviews, to understand the needs and challenges of each patient.
• Work collaboratively with patients to co-create individualized treatment plans that address their goals and respect their cultural backgrounds.
• Implement evidence-based therapeutic models such as Cognitive Behavioral Therapy (CBT), Dialectical Behavior Therapy (DBT), and Solution-Focused Brief Therapy (SFBT) in various formats including individual, couple, family, and group therapy sessions.
• Provide immediate crisis intervention services when necessary, and work with an interdisciplinary team of healthcare professionals to ensure comprehensive care during acute distress.
• Maintain detailed and accurate clinical documentation, including treatment plans, progress notes, and discharge summaries in compliance with clinic protocols and state regulations.
• Deliver psychoeducation to patients and their families about mental health conditions, equipping them with strategies and knowledge to manage their well-being effectively.
• Collaborate closely with a multidisciplinary team, including psychiatrists, psychologists, and case managers, to coordinate multifaceted patient care.
• Regularly assess treatment outcomes and adapt care plans based on feedback and clinical evidence, ensuring the highest standard of care.
• Stay abreast of current mental health research and practices to continually enhance your therapeutic techniques and approaches.
• Uphold ethical standards and protect patient confidentiality at all times.

You will be working with a diverse patient demographic that includes individuals dealing with trauma, grief, anxiety, depression, and other mental health challenges. By employing a trauma-informed lens, your practice will support survivors from various backgrounds, creating inclusive pathways to recovery and well-being.

• A Master’s degree in Social Work from an accredited institution.
• An active State license as a Licensed Clinical Social Worker (LCSW) is required.
• Proficient in both independent and collaborative work settings, demonstrating flexibility and adaptability as a part of a team.
• Exceptional communication and interpersonal abilities, essential for building rapport with clients from varied cultural backgrounds.

• Prior experience in trauma-informed care and working in hospice or palliative care settings is a significant advantage.
• Familiarity with electronic health record (EHR) systems to maintain clinical documentation efficiently.
• Experience in providing group therapy effectively, highlighting an ability to engage diverse client groups.
