YOUR ROLE AS A LONG TERM APP:
As a key member of a multidisciplinary team, you play a central role in patient care. You'll spend your days managing patients with acute and chronic diseases, collaborating directly with the attending physician to provide the highest quality of care. Your expertise is crucial in effectively managing patients with debilitating chronic diseases, improving their quality of life. You are a natural collaborator, communicating and working seamlessly with the Director of Nursing, Social Workers, and other team members to ensure a cohesive and comprehensive approach to care. This dynamic role keeps you engaged and connected as you make a tangible difference in the lives of those you serve.
OUR IDEAL PRACTITIONER
  • A board-certified geriatric, family, or adult Nurse Practitioner (ANCC, ACHPN, or AANP)
  • 1+ years of experience in post-acute/SNF/LTC
  • Prior post-acute, long-term, and/or skilled nursing facility experience
