What is a post acute care job?

Post acute care jobs refer to healthcare positions that provide medical and supportive care to patients recovering after hospitalization, surgery, or serious illness. These roles can be found in settings like skilled nursing facilities, rehabilitation centers, home health, and long-term acute care hospitals. Professionals in post acute care help patients transition from hospital to home or another facility, focusing on recovery, rehabilitation, and preventing readmission. Common roles include nurses, physical therapists, occupational therapists, and social workers. The goal is to help patients regain independence and improve their quality of life.

To thrive in a post-acute care role, you need a foundation in clinical care, care coordination, and knowledge of recovery and rehabilitation processes, often supported by a relevant healthcare degree and licensure (such as RN, LPN, or PT). Familiarity with electronic health records (EHRs), case management software, and regulatory compliance systems is crucial.
